Registered Nurse (RN) - PRN (As Needed)

McLeod Home Health is seeking compassionate and skilled Registered Nurses (RNs) to provide high-quality patient care in the home setting. This PRN opportunity offers flexibility while allowing you to deliver individualized care, collaborate with an interdisciplinary team, and help patients achieve optimal health outcomes in the comfort of their homes.
Position Summary
As a Home Health Registered Nurse, you will be responsible for assessing, planning, implementing, and evaluating patient care services while utilizing remote care technology and evidence-based nursing practices. You will work independently in the field while collaborating closely with physicians, therapists, nursing staff, and support teams to ensure coordinated, patient-centered care.
Key Responsibilities
  • Perform comprehensive patient assessments and develop individualized plans of care.
  • Provide skilled nursing care and monitor patient progress toward clinical goals.
  • Supervise and direct Licensed Practical Nurses (LPNs) and Home Health Aides (HHAs).
  • Collaborate with an interdisciplinary team including RNs, LPNs, PTs, OTs, SLPs, Social Workers, and support staff.
  • Utilize remote care technology to support positive patient outcomes.
  • Educate patients and caregivers on disease management, medications, and treatment plans.
  • Maintain accurate and timely clinical documentation.
  • Adhere to agency policies, regulatory standards, and professional nursing practice guidelines.
  • Travel to patient homes, including rural service areas, to provide care as needed.
  • Demonstrate exceptional customer service and communication skills with patients, families, physicians, and colleagues.

Experience
  • Minimum of 2 years of nursing experience preferred
  • Home Health experience strongly preferred
  • Strong assessment, critical thinking, and clinical skills required

Education & Licensure
  • Current South Carolina Registered Nurse (RN) license or Compact Multi-State RN License
  • Current BLS Certification (American Heart Association)
  • Valid South Carolina driver's license
  • Current automobile insurance as required by law
  • Must meet McLeod Health driving eligibility requirements
  • Must be at least 21 years old to be covered under McLeod Health's fleet vehicle insurance policy
