Job description

Hospice Registered Nurse

Location: Rutland, Vermont
Pay Rate: $65/hr
Contract: 13 weeks
Schedule: Tuesday–Saturday, 8:00 AM – 4:00 PM
Guaranteed Hours: 37 hours per week
Experience: Minimum 2 years required

About the Role

We are seeking an experienced Hospice Registered Nurse for a 13-week quick-start contract opportunity in Rutland, Vermont. This role serves as a hospice case manager, providing comprehensive patient assessments, individualized care planning, symptom management, patient and caregiver education, and coordination with the interdisciplinary care team.

Responsibilities

• Provide hospice nursing care and comprehensive assessments for assigned patients
• Develop, update, and communicate individualized plans of care with patients, caregivers, and the IDT
• Manage caseloads, visit schedules, pain control, symptom management, wound care, IV care, and high-tech needs
• Educate patients and caregivers on medications, treatment options, discharge instructions, and reportable conditions
• Document care accurately using McKesson and coordinate safe transfers, discharges, and supervisory visits

Requirements

• Active Vermont Registered Nurse License Required Upon Acceptance
• Minimum 2 years of Hospice RN, Home Health, or Case Management experience required
• Strong hospice assessment, care planning, symptom management, and patient education skills required
• McKesson experience preferred
• Must be available for Tuesday–Saturday schedule, some on-call, and holiday coverage
