Job description

Join a travel Physical Therapist role in Russellville, KY (Home Health) where you will help patients recover from injuries and manage chronic conditions while experiencing life in a new location. Minimum Requirements

  • Associate's, Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA. (Client minimum: Associates Degree accepted.)
  • Active Kentucky Physical Therapist license required to start the assignment.
  • Home health experience required. Must be proficient with HCHB electronic documentation.
  • Experience performing OASIS assessments, admissions, recertifications, 30-day assessments, and discharges.
  • Experience supervising PTAs.
  • BLS certification from AHA or ARC may be required.
  • Preferred: prior experience working in Logan and Todd county coverage areas.


Assignment Details

  • Facility Type: Home Health
  • Shift Name: Days
  • Shift Schedule: Monday - Friday, approximate daytime schedule (am to 4:30 pm) as indicated by the client.
  • Weekly Guarantee: 32 hours per week.
  • Productivity Expectation: 30 points.
  • Coverage Areas: Logan County and Todd County, KY.
  • Mileage reimbursement between patient visits from first to last patient: Yes.
  • On-call/Weekend Requirement: No.
  • Dress Code / Scrub Color: Royal blue.
  • Floating Requirement: Candidate may be required to cover assigned geographic areas as needed.
