About Elaris Home Care

Elaris Home Care is a compassionate, client-centered home care agency dedicated to helping seniors and adults remain safe, comfortable, and independent in their own homes. We are seeking an experienced Registered Nurse to join our team on a PRN contract basis to conduct nursing assessments and provide clinical oversight as needed.


Elaris Home Care is basednin Decatur, GA. We proudly serve DeKalb, Fulton, Cobb, Gwinnett, Clayton, Henry, Rockdale, Fayette, Cherokee, and Forsyth counties, delivering personalized care that helps seniors and adults live safely and independently in the comfort of their own homes.


Responsibilities

  • Conduct initial in-home nursing assessments for new clients.
  • Develop and update individualized plans of care.
  • Perform supervisory visits for caregivers in accordance with Georgia regulations.
  • Evaluate client health status and identify changes requiring intervention.
  • Collaborate with the Administrator and care team regarding client needs.
  • Provide caregiver education and clinical guidance when appropriate.
  • Complete accurate, timely, and compliant documentation.
  • Communicate with physicians and other healthcare professionals as needed.


Qualifications

  • Active Georgia Registered Nurse (RN) license in good standing.
  • Minimum of 2 years of nursing experience (home care, home health, hospice, or geriatrics preferred).
  • Current CPR certification.
  • Valid driver's license, reliable transportation, and auto insurance.
  • Strong assessment, communication, and documentation skills.
  • Ability to work independently and manage a flexible schedule.

  • Competitive per-visit or hourly compensation (based on experience and assignment type).
