Roles & Responsibilities:

• Conduct patient evaluations to develop individualized treatment plans.
• Implement treatment plans that promote patient recovery and rehabilitation.
• Monitor and document patient progress regularly.
• Collaborate with multidisciplinary teams to enhance patient outcomes.
• Educate patients and families about exercise, mobility, and health management.

Requirements:

• Licensed Physical Therapist or equivalent
• Relevant certification(s) as required by the role
