Job description


Responsible for working alongside multiple physician and health care professionals and facilitating advanced primary and specialty nursing services. Provides general and preventative care for adults at the end-of-life, treat illnesses, discuss goals of care, order lab tests, and prescribe medication in the inpatient and outpatient/ambulatory setting.
Principal Accountabilities:
1. Provides new patient consultation and reviews health history.
2. Performs physical examinations and screening on patients.
3. Provides after-care/ongoing care information and resources to patients and their families.
4. Ensures patient records are maintained including test results, immunizations, condition changes.
5. Delivers quality, advanced practice care while complying with company policies, safety regulations, and productivity metrics.
6. Recommends further treatment options or future appointments if required.
7. Performs diagnoses health problems and provides medications.
8. Performs other related duties as assigned.
Qualifications
Required:
1. Master's degree in Nursing in area of specialty practice; or post-master's certificate in area of specialty practice with graduate degree in nursing
2. Current state APN certificate
3. Current RN license.
Preferred:
1. 5 or more years experience in Nursing or relevant area.
