Schedule: Full Time | 40 hours per week | Monday - Friday | 8:00 am - 4:30 pm

What You'll Do In This Role:
Be a trusted helping hand in the moments that matter most. As a Home Health Concierge, you'll provide personalized, non-clinical support to participants-often one-on-one-in their homes or in care settings such as assisted living or skilled nursing facilities. Guided by the interdisciplinary team, you'll focus on enhancing comfort, independence, and overall well-being through meaningful, participant-centered support. If you enjoy building connections and making a daily difference in someone's life, this role offers a rewarding and impactful experience.
Key Responsibilities
  • Provide One-on-One Support - Offer individualized supervision, respite care, psychosocial support, and companionship.
  • Assist with Daily Living - Support meals, housekeeping, laundry, and transportation needs.
  • Support Medication Adherence - Remind participants and ensure compliance with care plans.
  • Engage Participants - Facilitate activities and use redirection techniques as needed.
  • Coordinate Transportation - Ensure safe transport and monitor bus arrivals/departures.
  • Assist with Medical Appointments - Support participants during on-site and community-based appointments.
  • Document and Communicate - Record participant interactions accurately and relay relevant information to the care team.

Skills, Knowledge & Expertise
  • At least one year of experience working with frail elderly or long-term care populations. High School diploma or GED, required
  • Reliable transportation and a valid Michigan Driver's License.
  • CPR certification or willing and able to obtain as offered by LifeCircles.
  • Strong desire working within a collaborative, team-based model of care.
  • Comfortable working independently and managing a self-directed workload.
