You will have access to advanced electronic medical records (EMR) systems that facilitate seamless patient documentation and care coordination.

• Establish and maintain rapport with patients and their families to effectively assess and address their needs, employing active listening and clear communication styles that resonate with diverse populations.
• Collaborate with an interdisciplinary treatment team—including behavioral health and primary care providers—to formulate comprehensive care plans that address both physical and mental health needs.
• Conduct evidence-based screenings for depression, anxiety, substance abuse, and other behavioral health concerns, facilitating appropriate follow-up assessments and referrals when necessary.
• Maintain a manageable caseload of patients, providing timely follow-up, brief interventions, and consistent support throughout their hospice experience.
• Utilize the electronic medical record system to document patient interactions and interventions within 48 hours, ensuring compliance with healthcare documentation standards.
• Develop and implement integrated care plans that detail necessary referrals and services, coordinating input from patients and their families to secure optimal outcomes.
• Safeguard patient privacy and confidentiality by conforming to applicable laws and best practices in documentation and data handling.
• Actively participate in professional development activities such as supervision, case conferences, and quality improvement meetings to enhance the quality of care provided.
• Assist in connecting patients with community resources relevant to their individual goals, encompassing educational, vocational, cultural, and spiritual support services.
• Respond to inquiries from patients and other healthcare providers regarding appointments and care coordination.

Your primary focus will be on patients within a hospice setting, often facing complex end-of-life issues. This includes understanding the psychosocial dynamics of grief and loss, as well as offering support that aligns with patients' and families' cultural and individual preferences. You will bring compassion and expertise to help navigate these sensitive situations effectively.

• Master's degree in a related Behavioral Health field.
• Valid Licensed Clinical Social Worker (LCSW) designation is required.
• Certification in CPR or BLS is preferred.
• A minimum of three years of professional experience in behavioral health, particularly in a clinical or social service setting.
• Strong cultural competence working with underserved communities or individuals experiencing homelessness.
• Experience applying trauma-informed and social justice frameworks in practice.
• Proficient in information technology; adept at utilizing electronic documentation and communication systems.
• Bilingual in English and Spanish is preferred but not required.

• Experience within a hospice or palliative care context, understanding the specialized needs of patients facing terminal illnesses.
• Prior involvement in telehealth services or working remotely is advantageous.
• Familiarity with population health management strategies and integrated care models to enhance service delivery.
