Purpose:

To provide support and assistance to individuals with intellectual and developmental disabilities to enable them to live valued lives in the community.


Qualifications:

  • Must successfully complete the required pre-employment Background and Drug Screenings
  • Must possess a valid Driver's License which meets company standards for approved driver status
  • Must possess advanced computer skills
  • Must possess a Bachelor's Degree in Human Services
  • Minimum two years of Management experience working in the field of intellectual/developmental disabilities preferred
  • Minimum two years of experience in writing program plans, case notes or other relevant documentation required

Essential Functions

  • Provides oversight and supervision to QIDPs and Program Coordinators in the development, implementation and coordination of each person's program plan
  • Ensures standardization and execution of programming documentation for QIDPs and Program Coordinators
  • Conducts and creates standardized trainings for QIDPs, Program Coordinators, Residential Program Directors, Directors of Operation and Executive Directors
  • Ensures that plans are person centered and reflect the needs and desires of the people supported by Sunrise
  • Verifies that communication and collaboration with all stakeholders, including family, guardians and support coordinators is taking place to address the needs of the people supported by Sunrise
  • Creates positive and collaborative communication with all disciplines and team members
  • Conducts audits of electronic programming documentation to validate that all facets are properly completed as required by organizational and regulatory guidelines based on the services provided
  • Ensures that those working in Waiver programs understand the advocacy process so that each person supported receives the necessary levels of services based on his/her needs
  • Oversees that program caseloads are balanced to optimize efficiency and quality documentation
  • Performs other duties as assigned
