Below, you'll find other important information about this position.The Post-Acute Care Services Intern provides strategic, operational and analytical support across Home Health, Hospice and Home Medical Equipment (HME). This role supports key initiatives, data analysis and program development to enhance quality, access and cost-effective care delivery across Post-Acute Care Services.

MINIMUM QUALIFICATIONS:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1. Currently enrolled in a graduate program (Master of Public Health, Master of Health Administration or related study).

1. Support strategic initiatives across Post Acute Care Services, including Home Health, Hospice, and Home Medical Equipment (HME).

2. Assists in the development of reports, dashboards and presentations for executive leadership.

3. Support the planning and implementation of innovative care models.

4. Participate in leadership meetings and assist with preparation of materials for strategic planning and decision making.

5. Collaborate with stakeholders to support care coordination, access and network development initiatives.

6. Assist with quality, regulatory, and compliance activities, including data tracking and audit preparation.

7. Support outreach and program development efforts to enhance awareness and utilization of post-acute services.
