Job description

The Complex Case Manager manages a cohort of Medicare Advantage patients to help ensure they receive the "right care at the right time" to improve clinical outcomes and lower costs. Utilizing an evidence-based approach the Complex Case Manager will optimize care to determine what is medically necessary, while eliminating excessive treatment and expense. The Complex Case Manager will act as mentor and coach to assigned Complex Care Coordinators within the department.

Requirements:

โ€ข Utilization knowledge on Part A, Part B and part D Medicare.

โ€ข Medicare Advantage experience required

โ€ข Knowledges of HEDIS

โ€ข Case Management experience

โ€ข Knowledge of the evidence based guidelines for the main chronic conditions-HTN, CAD, Heart Failure, Diabetes, CKD and COPD.

โ€ข Ability to understand CMS guidelines.

โ€ข Understanding of the basic principles for OSHA, CLIA, HIPAA and AHCA.

Duties and responsibilities:

ย โ€ข Works in collaboration and continuous partnership with chronically ill or "high-risk" patients and their family/caregiver(s) in a team approach to:

โ€ข Reduce emergency room utilization and hospital readmissions

โ€ข Promote timely access to appropriate care

โ€ข Increase utilization of preventative care

โ€ข Increase comprehension through culturally and linguistically appropriate education

โ€ข Create and promote adherence to a care plan, developed in coordination with the patient, primary care provider, and family/caregiver(s)

โ€ข Increase patientsโ€™ ability for self-management and shared decision-makingโ€ข Connect patients to relevant community resources, with the goal of enhancing patient health and well-being, increasing patient satisfaction, and reducing health care costs

Knowledge, Skills, and Abilities:

ย โ€ข Knowledge of Electronic Medical Records.

โ€ข Ability to read, comprehend, and interpret medical coding and medical documents.

โ€ข Ability to be collaborative and effectively communicate with patient.

โ€ข Ability to have strong sense of urgency and use judgment to respond to emergencies.

โ€ข Ability to solve practical problems and deal with a variety of situations.

โ€ข Ability to write simple correspondence

โ€ข Ability to effectively present information in one-one and small group situations to clients and other employees of the organization.

โ€ข Prefer bi-lingual in English and Spanish.

Education and Experience:

ย โ€ข Current State of Florida Registered Nurse

โ€ข Case Management experience required

โ€ข CCM preferred
