Position Overview

A Case Manager (CM)
is responsible for
assistingindividuals with chronic illnesses, mental health conditions, or life-altering challenges in gaining access to rehabilitation programs and essential community services. The goal of TCM services is tofacilitateaccess and linkage to the necessary resources that enable clients to live independently andmaintaina high quality of life.

TCMs serve as advocates, coordinating services and speaking on behalf of clients to ensure that they receive the mostappropriate careand support. The role involves direct interaction with clients, their families, healthcare providers, and other community resources.

Key Responsibilities

  1. Assessment & Planning

Conduct comprehensive assessments to evaluate clients' needs, strengths, challenges, and goals.

Develop individualized service plans tailored to each clients specific requirements.

Assess clients physical and mental wellness, preferences, and abilities to create a personalized care plan.

2. Service Coordination

Facilitate access tovarious services, including mental health treatment, housing, employment, education, and social support.

Connect clients with professional services, community resources, and relevant support programs.

Ensure that all services are aligned with the clients goals and best interests.

3. Advocacy & Support

Advocate for clients rights and needs, helping them navigate complex healthcare and social systems.

Represent clients in discussions with service providers to ensure they receiveappropriate careand support.

4. Monitoring & Evaluation

Regularly assess client progress and evaluate the effectiveness of services received.

Adjust treatment and support plans as needed to improve client outcomes.

Maintain consistent communication with clients to assess their well-being and address any concerns.

5. Collaboration

Work closely with clients, families, caregivers, healthcare professionals, and community agencies to provide holistic care.

Build strong relationships with local service providers to streamline client support.

6. Crisis Intervention

Provide immediate support and intervention during crises.

Assistclients in managing emergency situations and de-escalate conflicts when necessary.

7. Education & Empowerment

Educate clients and their families about available resources,their conditions, and treatment options.

Encourage clients to take an active role in their recovery and well-being.

8. Documentation & Follow-Up

Maintainaccuraterecords of client progress, referrals, home visits, and notable interactions.

Conduct periodic evaluations andmake adjustments tocare plans as needed.

Follow up with discharged clients to ensure continuity of care and satisfaction with services.

Qualifications & Requirements Education:

Bachelor's degree in a relevant field.

Certification in Targeted Case Management (TCM) meeting all requirements of the state of Florida.

Experience:

Minimum of one (1) year of experience in case management or a related field.

Skills & Competencies:

Bilingual candidates are a plus.

Strong knowledge of community resources and social services.

Excellent time management and organizational skills.

Strong communicationand documentation abilities.

Ability to work independently and collaboratively within a team.
