Position Title: Speech Language Pathologist

Supervisor: Director of Therapy

Department: Compass Community Health

FLSA Status: Exempt


POSITION SUMMARY:

The Speech Language Pathologist provides comprehensive differential diagnostic evaluations using formal and informal measures. Develops, implements and manages treatment programs for a variety of communication disorders through Compass Community Health. Also serves as a member of the interdisciplinary team.


ESSENTIAL FUNCTIONS:

Essential functions are duties, which are essential or primary to the position or the reason the position exists. An individual must be able to perform the essential functions of the position with or without reasonable accommodation.
1. Provides comprehensive differential diagnostic evaluations using formal and informal measures.
2. Provides management/treatment programs for individuals with communication disorders.
3. Provides appropriate case management/therapeutic intervention.
4. Provides appropriate written documentation of services provided.
5. Assumes responsibility for ancillary/priority services within department and contributes to development of department by participating in special projects/programs.


SECONDARY FUNCTIONS:

Secondary functions are duties, which are not exclusive of the position, and can be performed by other positions; however, secondary duties are to be performed for the efficiency of Compass Community Health.

  1. Assists with office and administrative tasks as needed.
  2. Performs any other duties as assigned by your supervisor in collaboration with the Clinic and Compliance Director, and Medical Director.
  3. Develop and sustain positive working relationships with patients, families, and providers.
  4. Explain to patients and family the roles and responsibilities of each team member and how they will work together to provide services.
  5. Remain fluent in terminology pertaining to a healthcare setting.
  6. Respect and respond to the leadership displayed by other providers in a healthcare setting or team.
  7. Facilitate collaborative care by sharing relevant information with others through communications that are authorized by the patient and are permissible under HIPPA and related laws, regulations, and policies.
  8. Assess the nature of the patient's family and social support system that have an impact on healthcare.
  9. Ensure the flow and exchange of information among the patient, family, and providers.
  10. Support patients in considering and accessing complementary and alternative services designed to support health and wellness.
  11. Provide information, education, guidance and support to family members and caregivers.
  12. Establish and pursue individual and team-based improvement goals.
  13. Participates in Patient Centered Medical Home team meetings and quality improvement activities.
  14. Participates in data collection, review of respective outcomes reporting, as well as programmatic clinical audits and evaluation related to Patient Centered Medical Home initiatives and other quality programs as appropriate.

MINIMUM QUALIFICATIONS, INCLUDING TRAINING AND EXPERIENCE:

  1. Requires masterโ€™s degree in Speech/Language Pathology; prefer experience with individuals with developmental disability.
  2. Must be Licensed by the Ohio State Board of Speech/Language Pathology and Audiology; and maintain certification as scheduled by Compass Community Health. May be required to obtain course work at own expense for initial certification as well as renewals.
  3. Prior experience working in patient/family health care environment preferred.
  4. Proficient in communication technologies.
  5. Previous experience with health IT systems, EHR, and health IT systems preferred.
  6. Previous experience working with a variety of populations.
  7. Effective organizational skills and ability to collect and maintain data required.
  8. Maintains applicable agency trainings.
  9. Must be able to bend, stoop, and lift up to 50 lbs. Must be able to be physically active throughout the workday.
  10. Valid Ohio Driver's license; must be able to transport self. Must be able to maintain insurability as required by Board policy.


PHYSICAL DEMANDS

While performing the duties of this job, the employee is regularly required to communicate effectively; use hands and fingers to handle or operate tools, equipment, or controls; and to reach, bend, and lift as necessary. The employee may be required to stand or sit for extended periods and occasionally move or lift up to 25 pounds. Specific vision abilities required by this job include close vision, distance vision, and the ability to adjust focus.
