Occupational Therapy Assistant (COTA) - Rock Rapids, IA

Blue Stone Therapy is seeking an Occupational Therapy Assistant (COTA) to provide rehabilitation services to adult and geriatric patients at Lyons Specialty in Rock Rapids, IA.


This role focuses on delivering skilled occupational therapy services to adult and geriatric patients in a skilled nursing and post-acute rehabilitation setting. The Occupational Therapy Assistant works under the supervision of a licensed Occupational Therapist to implement treatment plans designed to improve activities of daily living (ADLs), functional mobility, strength, cognition, and overall independence.


This position will serve multiple partner communities, including Sibley Specialty, with travel between locations as needed.


What You’ll Do

  • Provide skilled occupational therapy treatment in accordance with established plans of care
  • Implement individualized treatment programs focused on functional and measurable outcomes
  • Treat adult and geriatric patients in a skilled nursing facility (SNF) setting and rehabilitation settings
  • Monitor patient response to treatment and communicate progress with the supervising Occupational Therapist
  • Maintain timely, accurate documentation to support quality care and reimbursement standards
  • Collaborate with interdisciplinary team members and facility staff to support positive patient outcomes
Requirements:

What You Bring

  • Graduate of an accredited Occupational Therapy Assistant program
  • Current Occupational Therapy Assistant license (or eligibility for licensure) in applicable state
  • Experience in skilled nursing, long-term care, or post-acute rehabilitation preferred
  • Strong communication, organization, and clinical reasoning skills
  • Commitment to patient-centered care and professional standards
