Gotham Enterprises Ltd is seeking a dedicated Senior Social Worker to join our passionate team at our Community Center in Augusta, Georgia. This full-time remote position focuses on providing telehealth support to our senior clients, ensuring they receive tailored mental health care and psychosocial assistance. The successful candidate will play a crucial role in enhancing the quality of life for seniors and their families.

Work Environment

This role is fully remote, leveraging telehealth technologies to provide critical support to seniors in a comfortable and accessible setting. Our team employs advanced telecommunication tools to ensure effective interaction with clients and their families. The work hours are generally Monday to Friday from 8:00 AM to 4:30 PM, with some flexibility required for evening or weekend hours based on client needs. On-call availability may also be necessary to manage urgent situations.

Key Responsibilities
  • Conduct comprehensive assessments to identify the psychosocial needs of seniors, taking into account emotional, mental, and social factors.
  • Develop and implement personalized care plans in collaboration with clients and their families, addressing specific goals and preferences.
  • Provide emotional support and counseling to seniors and their families, assisting them with life transitions such as aging, loss, or chronic illnesses.
  • Advocate for clients’ access to community resources and services, ensuring they receive the necessary support systems in place.
  • Work with interdisciplinary teams, including healthcare professionals and caregivers, to provide coordinated care that addresses the holistic needs of clients.
  • Regularly monitor and evaluate clients' living conditions and well-being through scheduled virtual visits and address any concerns that arise.

Qualifications
