ESSENTIAL DUTIES AND RESPONSIBILITIESΒ include the following. Other duties may be assigned.Β 

Coordinates the needs of clients and family members with resources available and provides client advocacy as needed.Β 

AssistsΒ clients in finding and accessing community resources needed toΒ maintainΒ independent functioning.Β 

AssistsΒ clients, individually and in groups, with development of independent and community living skills, social skills, interests, and leisure time activities which will encourage community involvement and integration.Β 

AssistΒ clients in developing an individualized recovery plan that focuses on personal and interactive problems, independent living skill needs, defining goals, andΒ to planΒ action reflecting interests, abilities, and needs.Β 

Establishes rapport with clients, assessing client needs; organizes,Β plans,Β and delivers face-to-face client services; and conductsΒ follow upΒ on the outcome of services provided.Β 

May provide services through individual or group rehabilitation to address symptoms through use of curriculum-based education and skills practice toΒ assistΒ clients in developing skills necessary to perform daily living and successful integration into community life.Β Β 

Adheres to agency policy and procedures, as found in the Forms Locked drive on each computer.Β Β Also adheres toΒ CARF and DMHSAS standards and criteria, federal and state confidentiality rules and regulations, and any special funding source requirementsΒ including Medicaid and HCA regulations.Β 

Supports and promotes a service environment for individualsΒ in need of behavioral health and/or co-occurring services thatΒ isΒ recovery focused, welcoming, and attentive to the needs of individuals who may have experienced trauma in their lives.Β Β Attends annual core competencies trainingΒ inΒ relationshipΒ toΒ individuals with co-occurring disorders or who have experienced traumaΒ in their lives.Β 

Establishes andΒ maintainsΒ professional working relationships withΒ supervisorΒ and peers.Β Β CanΒ effectively work within a team or group setting cohesively.Β 

Monitors eligibility status of clients for Social Security, Title XIX (Medicaid), QMB, Medicare and other benefits for which the client may qualify for andΒ seeksΒ toΒ assistΒ eligible clients in the Health Home Services/enrollment.Β 

Maintains up-to-date address, phone number, contact persons, and financial informationΒ onΒ all clients.Β 

Establishes rapport with clients, assessing client needs;Β organizes,Β plans,Β andΒ delivers face-to-face client services; and conductsΒ follow upΒ on the outcome of services provided.Β 

Responsible forΒ maintainingΒ strictestΒ confidentiality guidelines for interactionsΒ regardingΒ consumer and family information.Β 

Provide clinical services for at least 80% of caseload..Β 

Utilizes the philosophy of Collaborative/Therapeutic Documentation to meet clinical record keeping documentation standards and ensure documentation is completed with the Consumer to the extent possible and all documentation is completed on the same day of the date of service.Β Β Reports andΒ documentsΒ servicesΒ in accordance withΒ agency policy and procedures.Β 

Responsible forΒ seeking outΒ supervisoryΒ assistanceΒ forΒ additionalΒ training and to enhance skill set as needed.Β 

InteractsΒ with other professionals to discuss therapy or treatment,Β new resourcesΒ or techniques, and to share information.Β 

May beΒ requiredΒ to work evening or weekend hours.Β 

May beΒ requiredΒ to provide services in a community setting.Β 

MustΒ beΒ able toΒ provide client transportation using personal vehicle or agency owned van or small bus.Β 

Performs other related duties as assigned.Β

EDUCATION and/or EXPERIENCEΒ Β 

  • Must meet ODMHSAS and HOPE’s requirementsfor Behavioral Health Case Manager II certification.Β 
  • Case management experience in behavioral health or social services preferred.Β 

CERTIFICATES, LICENSES, REGISTRATIONSΒ Β 

Required to obtain and maintain DMHSAS CM Certification.Β 

Required to complete, Therapeutic Options or approved safetyΒ training and annual updates.Β 

May beΒ requiredΒ to obtain andΒ maintainΒ First Aid and CPR Certifications.Β 

May beΒ requiredΒ to complete andΒ maintainΒ Wellness Coach CertificationΒ 

In order toΒ provide client transportation,Β mustΒ possessΒ current, valid OklahomaΒ license, meetΒ HOPE insurance requirements.Β Β HOPE will annually obtainΒ copyΒ of Motor Vehicle Record from the Department of Public Safety.Β

Ability to write correspondence to courts, schools, etc. Ability to effectively present information in one-on-one and small group situations to families, clients, and other employees of the organization.Β Β Ability to complete thorough andΒ accurateΒ clinical documentation and writing in a medical record chart and complete needed paperwork.Β
