As a Social Worker, you will assess, resolve, prevent or lessen the impact of psychosocial,physicaland mental health related issues for clients and their families. Care will take place within a residential or institutional setting with referrals generated by a variety of sources including government agencies, employers, insurancecompaniesor private payees. Client groups receiving treatment may includepediatrics forin homeor school services and/oradults with medical/health concerns.

Whatyou'lldo:

  • Conduct client assessments related to financial, accommodation, legal or mental healthbarriers

  • Assistclients in setting S.M.A.R.T.goals

  • Design and implement treatment plan for clients usingevidence basedinformation, best practice, and clinicalexpertise

  • Work collaboratively with a multi-disciplinary team consisting of government agencies, healthcare workers, and other communityresources

  • Advocate on behalf of clients and families dealing with mental health or disabilities, chronic medical conditions, addictions, palliative and othersituations

  • Identifyand link clients and families to available resources within thecommunity

  • Offer health teaching to clients,familiesand caregivers as it relates to client's medicalcondition

  • Evaluate the effectiveness of interventions, client response, and progress toward goal achievement or for need to altergoals

  • Communicate with the Health Care teamregardingfindings,treatmentsandprogress

  • Maintainprofessional client records related to client encounters and plans in compliance withParaMed'spolicy and the standards applied by the regulatory body or college.

  • Maintain the knowledge,skillsand attitudes to provide safe, efficient, effective service in all areas of practice by keeping abreast of changes in practice setting that affect scope ofpractice

Whatyou bring:

  • Degree in Social Work combined with registration /licensing with provincial regulatory college

  • Minimum of 2 years' experience as a practicing Social Worker preferably in a community setting

  • Working knowledge of applicable legislation including Mental Health Act, Substitute Decisions Act, Healthcare ConsentActand others

  • Superior oral and written communication skills to effectively interact with clients and familiespossessingvarying abilities forunderstanding

  • Effective skills in critical thinking and problemsolving

  • Knowledge of applicable community resources available for clients and families

  • Current certification (within the past 12 months) of First Aid, CPR completed withinCanada

  • Able to provide a current, clear Vulnerable Position Screening (VPS)
