Job description

Center of Hope - Psychiatric Case Worker
Valley Health Partners, Allentown, PA
Job Summary:
Provides case management services; supports counseling, patient and family education, and crisis intervention; and provides utilization review and discharge planning services under the supervision of department leadership
Qualifications:
Education:
  • Bachelor's Degree in Social Work, Psychology, Counseling, or related mental health field. Relevant Master's Degree preferred.

Experience:
  • 2 years of experience in a mental health setting.

Training:
  • CPR / BLS upon hire

Essential Functions:
  • Provides case management services, including linkages among departmental and community resources, under the supervision of practice leadership.
  • Provides supportive counseling, acts as patient/family education and liaison, and assists with crisis intervention under the supervision of practice leadership.
  • Provides utilization review and discharge planning services using a comprehensive grasp of VHP community supports.
  • Assists with information gathering to support patient assessment, initial/ongoing treatment plan, or for ongoing treatment issues under.
  • Documents activities in a timely fashion and in adherence with program, departmental, and regulatory standards.
  • Takes on other duties as assigned
