Job description

MINIMUM QUALIFICATIONS
Education- A Bachelor's Degree is preferred.
Experience- High school diploma or equivalent, plus 6 months of direct, documented experience working with people with mental illness or substance abuse issues.
Licenses & Certification- Case Manager I Certification or eligibility for certification is required. Must possess a valid Oklahoma Driver's License and be insurable.
Knowledge & Skills:
  1. Knowledge social services programs and available community resources.
  2. Skill in interpersonal and human relations, including teaming and collaboration abilities.
  3. Skill in crisis intervention.
  4. Ability to work as a team member with other staff to achieve the goals and objectives of the program.
  5. Ability to follow Rehab/BHS guidelines, IHS guidelines and established Policies and Procedures.
  6. Ability to utilize a personal computer and its applications in a Windows environment.
  7. Ability to exercise sound independent judgment establishing work priorities.
  8. Ability to meet assignment deadline and interpret established policies and procedures.
  9. Ability to maintain strict consumer confidentiality at all times.
  10. Ability to maintain a confident, professional demeanor.
  11. Basic understanding of Rehabilitation programming, Integrated Care, Substance Use and Suicide
  12. Ability to work in an interdisciplinary setting.
  13. Ability to assess and determine a patient's needs.
  14. Willingness to learn and navigate the system, being a main point of contact for shared patients and families with the ability to confidently answer questions for patient and families.
  15. Other Case Management/Coordinator duties that may be assigned.
JOB SUMMARY
The purpose of the position is to ensure a holistic, whole-person approach to providing patient education, support, referrals and resource links. The Case Manager I will be responsible for assisting in milieu management of residents, maintaining contact and follow up care to patients identified.
WORK ENVIRONMENT
Work is performed in a rehabilitation program. Occasional travel may be required in the performance of duties.
PHYSICAL DEMANDS
Position requires the demands of rehabilitation program (sitting, standing, walking, lifting, and moving. Must be able to life 50 pounds. Possibly driving a GSA vehicle as required.
ESSENTIAL FUNCTIONS
Satisfactory job performance will be determined by successful execution of the following:
  1. Knowledge of tribal beliefs and customs necessary to courteously and effectively communicate with clients and their families, Rehab/BHS staff, outside agencies, and the general public both orally and in writing.
  2. Confers with clients to assess their individual and family needs for appropriate community referrals.
  3. Develop various community relationships and knowledge of community resources necessary to assist patient and families with identified needs.
  4. Ensure confidentiality and participant protection procedures are appropriately maintained.
  5. Encourage patient participation in treatment and scheduled activities.
  6. Assists in the orientation of new patients into the rehabilitation program.
  7. Assists in the monitoring and documentation of patient participation, direction, behaviors, compliance with schedule and rules, and relapse prevention.
  8. Possible monitoring UA's.
  9. Confer with staff, therapist and nurse regarding client needs.
  10. Regular attendance is required to perform the essential functions of this position.
  11. Be able to work in an interdisciplinary setting.
  12. Complete required interagency and other necessary paperwork.
  13. Willingness to be a team player to ensure success.
  14. Performs other duties as required.
