Job description

The Portage County Health & Human Services department is hiring a full-time Social Worker position to support the Community Programs division.
Pay range starts at $31.65 / hr
Summary of position:
This position participates in a recovery oriented, person centered team approach to maintain a comprehensive community support program for consumers with a diagnosis of a mental disorder and/or a substance use disorder. Its role is to ensure that the service plan and service delivery for each consumer is integrated, coordinated, and monitored, and is designed to support the consumer in a manner that helps the consumer to achieve the highest possible level of independent functioning. It provides psychosocial education, crisis intervention, service facilitation, and consultation services to individuals and their families and court-related services under Chapter 51 and 55. This position primarily serves consumers enrolled in our Adult Comprehensive Community Services (CCS) program.
What are the primary job duties?
Provides CCS Service Facilitation.
  • Complete comprehensive assessments for each CCS consumer in order to identify strengths, needs, values, priorities and recovery goals.
  • Develops recovery oriented service plans for CCS Consumers that address the needs and recovery goals that were identified by the consumer during the assessment process.
  • Monitor the service plan for progress toward goals and consumer satisfaction.
  • Provides coordination to community agencies for all consumers.
  • Facilitate regularly scheduled team meetings for the consumer and his/her recovery team.
  • Participates in arranging appropriate placements for consumers.
  • Records clinical treatment notes and progress notes according to program standards.
  • Prepares and presents consumer cases for clinical supervision.
  • Monitors inpatient treatment plans and develops discharge plans for assigned consumers in psychiatric facilities.
  • Provides consumer transportation when appropriate.
  • Initiates court proceedings, arranges witnesses, testifies at hearings, completes court reports and makes recommendations to the court for appropriate treatment and facilities as relates to Chapter 51 and 55.

Provides Crisis Intervention Services Relative to Chapter 51 and 55 Proceedings.
  • Conducts interviews with consumers, family, law enforcement, medical personnel, and others as appropriate to assess the nature of the issue.
  • Assesses situation and determines appropriateness of referral to Corporation Counsel office for legal action.
  • Screens information and prepares dispositional recommendations to Corporation Counsel office.
  • Attends and provides testimony at commitment hearings.
  • Provides consultative services to family members before and after hearings regarding process, options, and results.
  • Provides case management and follow-along services to post-commitment consumers.
  • In conjunction with treatment staff, formulates recommendations for dismissal or recommitment to Corporation Counsel.

What are the minimum qualifications?
  • Bachelor degree in social work or any other human services field required.
  • One year experience within the last ten years working with persons with a diagnosis of a mental disorder required.
  • Certification or eligible for certification as a Social Worker in Wisconsin preferred, but not required.
