Job Title: Community Care Program Manager

Location: Park Ridge, IL — On-site with field travel
Pay: $55,000 – $72,000 per year, based on experience

Job Summary:

The Community Care Program Manager oversees the coordination and delivery of community-based care services for a caseload of clients receiving home care and supportive services under Illinois community care programs. This role sits between direct care and senior operations leadership — managing care coordinators, ensuring compliance with program requirements, and serving as the quality and accountability point for a defined service region. The position is well suited for someone with a clinical or care coordination background who is ready to step into a supervisory management role.


Key Responsibilities:
  • Oversee a team of care coordinators and field staff delivering community care program services to assigned clients
  • Ensure compliance with Illinois Department on Aging, Medicaid waiver, and CCP program requirements
  • Conduct client assessments, care plan reviews, and home visits as required by program standards
  • Monitor service delivery quality and address gaps, complaints, or compliance issues in a timely manner
  • Manage caseload assignments and scheduling to ensure adequate coverage and continuity of care
  • Maintain accurate documentation and reporting in compliance with state and agency requirements
  • Serve as a point of escalation for care coordinators, clients, and family members with complex needs

Must-Haves / Requirements:
  • 2+ years of experience in home care, community care coordination, or a related social services or healthcare role
  • Familiarity with Illinois community care programs, Medicaid waiver services, or aging services
  • Prior supervisory or lead experience overseeing direct care or coordination staff
  • Strong organizational and documentation skills with experience in care management platforms
  • Valid driver’s license and ability to conduct field visits as needed
