Job description

Yee Hong Centre for Geriatric Care (Yee Hong) is a recognized leader in senior care, dedicated to providing exceptional, culturally inclusive services that enhance the well-being of older adults. With a strong reputation for excellence in long-term care, community programs, and aging-in-place initiatives, Yee Hong is committed to supporting seniors in living with dignity, independence, and the best possible quality of life. At the core of its mission is a focus on compassionate, person-centered care tailored to the unique needs of diverse communities.

Yee Hong is one of the foremost providers of culturally appropriate senior care in the country, delivering high-quality resident experience and care to Chinese and other Asian seniors since 1994. The organization operates 805 long-term care beds and 308 seniors' apartment units, with an additional 224 long-term care beds opening in a new home by June 2026. In addition, Yee Hong offers four campuses of managed care, a hospice residence, and a wide range of community-based senior and caregiver support services. Through this comprehensive network, the organization serves more than 15,000 individuals annually across the Greater Toronto Area.

In consultation with the Program Manager, Department Head of Community Services or delegate, plans, coordinates, implements and evaluates programs to enable clients to adjust to program, cope with illness, promote and regain confidence and self-worth, maintain maximum functioning and independence.

Responsibilities:

  • Designs, plans, coordinates, implements and evaluates programs to enable clients to cope with illnesses, promoting and regaining client's confidence, self-worth and maintaining and maximizing independence.

  • Coaches, develops, and promotes an engaged, dynamic team. Contributes to the evaluation of staff's performance

  • Works closely with client, worker, physician and caregiver in assessing eligibility for service and ensuring appropriate design of care plan and ongoing monitoring of plan.

  • Ensures provision of personal care and administration of medication in accordance with the legislation

  • Assists in the development of program budget and implements effective expenditure control system for the Program

  • Provides timely and professional supportive counselling, information and education to clients and caregivers

  • Ensures implementation of risk management policy and safe and hygienic environment for clients and staff

  • Coordinates the optimal deployment of volunteers for the program and provides effective guidance, training and support to the volunteers

  • In consultation with the Manager, develops and updates policies, procedures for the program

  • Ensures efficient and responsible use of resources and maintains accurate record of fees and expenditures

  • Maintains up to date and accurate files, records and statistics on the program

  • Builds up and maintains an up to date resource library of relevant educational material

  • Keeps up-to-date with developments and best practices in the field

  • Other duties as assigned

Qualifications:

  • Bachelor degree in a relevant health or social services discipline or equivalent. Regulated health professional an asset
  • Minimum 2 years experience in social services, supervisory experience an asset
  • Previous experience working with the geriatric population an asset.
  • Recent CPR certification (completed within one year from date of hire)
  • Sound computer application knowledge
  • Excellent customer service skills
  • Excellent organizational and time management skills
  • Demonstrated ability to take initiative to independently problem solve using sound judgement.
  • Recent satisfactory Police Vulnerable Sector Screen result
  • Ability to communication in English and Chinese
