NorthKey Community Care is seeking a Community Integration Care Coordinator. The ACT/AOT Care Coordinator serves as a member of the multidisciplinary Community Integration Team, providing community-based care coordination, outreach, rehabilitation support, case management, and recovery-oriented services to individuals with serious mental illness and co-occurring disorders. Services are delivered through Assertive Community Treatment (ACT), Assisted Outpatient Treatment (AOT/Tim's Law), and other community-based programs to promote recovery, stability, independence, and successful integration within the community. WORK LOCATION: Varied as Assigned (Community-Based Services, Client Residences, Hospitals, Courts, Correctional Facilities, Personal Care Homes, and Other Community Locations) General Hours: Monday through Friday 8:30 am to 5 pm.

QUALIFICATIONS

• Bachelor's degree in Social Work, Psychology, Human Services, Sociology, Behavioral Science, or related field required. 
• At least one year of experience in behavioral health, substance use treatment, case management, rehabilitation services, or community support services required.
• Experience working with ACT, AOT, forensic, or community behavioral health populations preferred.
• Valid driver's license, insurance, acceptable driving record, and reliable transportation required.

JOB DUTIES

• Engage clients in a structured, collaborative relationship that promotes recovery, self sufficiency, independence, and community integration. 
• Provide community-based services in homes, hospitals, courts, correctional facilities, shelters, and other community settings.
• Participate as an active member of the multidisciplinary ACT/AOT treatment team.
• Coordinate medical, behavioral health, housing, employment, educational, transportation, and social support services.
• Assist clients in obtaining and maintaining benefits, insurance coverage, and community resources. 
• Provide outreach services to individuals who are difficult to engage in traditional clinic-based treatment. 
• Maintain frequent face-to-face contact with clients in community settings.
• Support clients in developing daily living, independent living, social, vocational, and community functioning skills.
• Coordinate and support services for individuals with serious mental illness, co occurring disorders, substance use disorders, or developmental disabilities.
• Identify and address risks and gaps in care that may contribute to hospitalization, emergency room utilization, housing instability, or reduced functioning.
• Provide education to clients, families, and natural supports regarding recovery, wellness, and available community resources.
• Assist with crisis response, safety planning, and coordination of appropriate services in collaboration with the treatment team.
• Provide emergency after-hours on-call coverage during scheduled after-hours shifts. Responsible for answering and responding to after-hours phone calls regarding program and client concerns and providing an onsite response when indicated.
• Coordinate care during transitions between hospitals, correctional facilities, residential settings, and community-based services.

ACT, AOT, and Criminal Justice Coordination

• Provide treatment monitoring and support services for individuals participating in Assisted Outpatient Treatment (AOT).
• Support compliance with Tim's Law and court-ordered treatment requirements.
• Collaborate with courts, probation, parole, detention facilities, attorneys, law enforcement, and other stakeholders.
• Track court orders and treatment participation requirements.
• Attend court hearings, case reviews, staffing meetings, and community partner meetings as assigned.

Documentation and Quality Assurance

• Complete all required documentation accurately and within established timelines.
• Complete reports, referrals, prior authorizations, and other required records in a timely manner.
• Monitor and document progress toward treatment plan goals.
• Monitor service utilization and assist in maintaining reimbursable services.

Professional Responsibilities

• Complete all mandatory training and professional development requirements.
• Maintain HIPAA compliance and client confidentiality.
• Participate in daily ACT team meetings, multidisciplinary staffing, and program meetings.
• Participate in weekly AOT meetings, multidisciplinary staffing, and program needs.
• Participate in on-call rotations and after-hours coverage as assigned.
• Perform other duties as assigned.

In Relation to Administration 

• Adhere to all NorthKey Community Care policies and procedures.
• Maintain confidentiality in all matters pertaining to client information.
• Maximize efficiency and effective use of time while maintaining accurate and punctual time reporting.
• Attend internal meetings related to responsibilities and duties.
• Maintain a professional and business-like manner when interacting with staff, community partners, courts, hospitals, and referral sources.
• Represent NorthKey Community Care in a professional and appropriate manner.
