Community Care Relations & Field Support Specialist – Grundy & LaSalle Counties

Job Description
Our company helps seniors who are living with chronic illnesses to LIVE OUT LOUD — to wake up each day feeling valued, supported, and empowered, no matter what their medical diagnosis says. You know those seniors who think life has slowed down for good? Yes, those ones. We help them get stronger, regain confidence, and rediscover joy in everyday living.

Because we’re expanding to Grundy and LaSalle Counties with a brand-new office in Morris IL, we’re looking for a versatile, dynamic, and relationship-driven team member who thrives on connection, compassion, and purpose.

Our clients are retired executives, professionals, and families who value excellence and integrity. Our mission is to redefine aging in place through nurse-practitioner–led care that promotes independence, preserves dignity, and eases the burden of caregiving.


Key Responsibilities

1. Relationship Building & Community Outreach
Serve as the local face of our company—attend networking events, community fairs, and senior resource meetings to build trust and visibility in Grundy and LaSalle counties.

2. Recruitment & Onboarding Support
Assist with hiring, interviewing, and onboarding new caregivers to ensure we build a strong, compassionate Morris team.

3. Quality Oversight & Field Support
Support clients and caregivers during the first weeks of service to ensure smooth transitions and high-quality care experiences.

4. Administrative & After-Hours Support
Help with scheduling, documentation, and communication; participate in the on-call rotation (one week every 4–6 weeks).

5. Client Onboarding & Start of Care (When Needed)
Meet new clients in their homes, complete intake assessments, set expectations, and launch care services with warmth and professionalism


The Ideal Candidate

Requirements
  • Valid driver’s license and reliable transportation

  • 2 years experience ih home health care and/or CNA certification

  • Must pass background, alcohol, and 10-panel drug screening

  • Comfortable using technology for scheduling and communication

  • Availability to work flexible hours, including occasional evenings or weekends for events
