Job description

Provide direct support to patients and families affected by violence, including coaching, mentorship, emotional support, resource referrals, and advocacy.
Assist in the development and implementation of treatment care plans for patients, focusing on their unique needs and circumstances.
Collaborate with hospital staff, community organizations, and other stakeholders to ensure a comprehensive and coordinated approach to violence intervention.
Conduct assessments to identify risk factors and develop strategies to mitigate them.
Provide education and training to hospital staff on trauma-informed care and violence prevention.
Participate in community outreach efforts to raise awareness about violence prevention and available resources.
Maintain accurate and confidential records of patient interactions and interventions with proper and prompt documentation.
Required:
High school diploma or GED, or ability to complete GED within 1 year of hire required
Preferred:
  • Associate's Degree in Social Work, Psychology, or a related field preferred but not required.

  • Basic computer proficiency email, documentation systems, and data entry.

  • Experience performing social services case management preferred

  • Previous experience working in a healthcare setting or with trauma survivors is a plus.

Required:
  • Obtain certification in one or more of the following (or equivalent certifications) within 1 year of hire: SPARCS - Structured Psychotherapy for Adolescence Responding to Chronic Stress

  • CTRP - Certified Trauma & Resilience Practitioner Peer Mentor Certification

  • Street Outreach Certification Preferred
