SUMMARY As a Community Health Nurse, you will focus on providing nursing interventions, patienteducation, and care management to a variety of clients in our community. Location: Rochester Regional Health Home Care - Monroe County Team
Hours Per Week: 30 paid for 40
Schedule:Weekend, Saturday/Sunday and 1 other consistent day during the week.*Work 30 hours, get paid for 40 - enjoy full-time pay with a reduced schedule*

RESPONSIBILITIES
  • Patient Care & Service. Provide comprehensive assessments of the bio-psycho-social needs of acutely ill clients in their home; perform skilled nursing care and prescribed treatments to clients based on MD orders; provide client and family education in accordance with client assessment and plan of care
  • Planning. Develop and document individualized care plans customized for each patient's unique needs, with support from the interdisciplinary health team as needed; maintain effective communication to convey patient health status, treatment plans and progress
  • Communication. Maintain effective communication to convey patient health status, treatment plans and progress; communicate with office and supervisory staff regularly; coordinates client services and/or referrals in an efficient and timely manner
  • Documentation. Accurately and completely document client care; document and implement physician orders per D.O.H. and program standards
  • Compliance. Adhere to required department and system protocols, regulations (local, state, federal) and education requirements
PREFERRED QUALIFICATIONS:
  • Compassionate, independent ,and patient focused
  • Comprehensive nursing assessments and documentation, decisive judgment and professional behavior
  • Two (2) years experience in home care preferred
  • Bilingual preferred
  • At least one year of nursing experience
REQUIRED QUALIFICATIONS: Diploma or Associate's Degree in Nursing required; Bachelor's Degree in Nursing preferred. Registered Nurse license in New York State.
Valid NYS Driver's License.

PHYSICAL REQUIREMENTS:

M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
