Job description

The Agricultural Community Outreach Specialist provides activities and programs that improve the health and overall quality of life of community residents by decreasing barriers that may prevent them from accessing health and medical care; bringing educational messages about health and CSC into the community and engaging residents in improved individual health. Duties include door-to-door outreach, community/neighborhood canvassing; successful medical home and social service referral; Increasing public education and awareness; the accurate reporting of information and data; positive public relations and integration of community services into CSC’s current delivery of care.


Education and Experience

  • High school diploma or GED equivalent is required
  • One to three years of experience in the field is required
  • Strong interpersonal and communication skills and the ability to work effectively with a range of constituencies in a diverse community.
  • Must read, write and speak Spanish fluently.
  • A valid driver’s license is required.
  • Ability to work independently and handle problems involving concrete variables in standardized situations.


Hours

Monday – Friday; 8:00 a.m. – 5:00 p.m.
