Key Responsibilities
  • Conduct in-depth psychosocial assessments of senior clients to identify their emotional, social, and mental needs.
  • Collaborate with seniors and their families to formulate personalized care plans that consider their individual goals and preferences.
  • Deliver counseling and emotional support to clients, assisting them in navigating life transitions, grief, and other challenges.
  • Advocate for clients by connecting them to appropriate community resources, ensuring they receive necessary support services.
  • Work closely with interdisciplinary teams, including nurses, caregivers, and healthcare professionals, to coordinate comprehensive care strategies.

Patient Population / Client Focus

Your role primarily focuses on seniors facing various challenges associated with aging and end-of-life care. Qualifications
  • A Masterโ€™s degree in Social Work from an accredited institution.
  • Current state licensure as a Licensed Clinical Social Worker (LCSW).
