Celestial Family Therapy is seeking a dedicated Licensed Clinical Social Worker (LCSW) to join our community-centered team in Naperville, Illinois. • Conduct in-depth psychosocial assessments of senior clients to identify their emotional, social, and mental needs.
• Collaborate with seniors and their families to formulate personalized care plans that consider their individual goals and preferences.
• Deliver counseling and emotional support to clients, assisting them in navigating life transitions, grief, and other challenges.
• Advocate for clients by connecting them to appropriate community resources, ensuring they receive necessary support services.
• Work closely with interdisciplinary teams, including nurses, caregivers, and healthcare professionals, to coordinate comprehensive care strategies.
• Perform regular home visits to monitor clients’ physical and emotional well-being while addressing safety concerns and living conditions.
• Facilitate communication among family members and resolve conflicts related to care and support arrangements.
• Educate clients and families about available community resources, support groups, and programs that enhance social engagement and living quality.
• Maintain accurate documentation of client assessments and care plans in compliance with ethical standards and regulations.
• Stay current with best practices in social work and participate in ongoing training and development opportunities.

Your role primarily focuses on seniors facing various challenges associated with aging and end-of-life care. You will engage with clients experiencing emotional distress, cognitive impairments, and social isolation, providing holistic support tailored to their diverse needs. By addressing both the psychosocial and practical aspects of their situations, you will help enhance their quality of life and ease their transitions.

• A Master’s degree in Social Work from an accredited institution.
• Current state licensure as a Licensed Clinical Social Worker (LCSW).
• At least two years of hands-on experience in a social work role, preferably in a geriatric or hospice setting.
• Comprehensive understanding of social work principles, aging-related issues, and available community support resources.
• Strong assessment, therapeutic, and problem-solving skills.
• Ability to build rapport and show empathy toward seniors and their families.
• Excellent communication and interpersonal skills for effective collaboration within interdisciplinary teams.
• Proficiency in electronic health record (EHR) systems and relevant computer applications for accurate documentation.

• Experience in hospice or palliative care settings.
• Experience in case management and community resource navigation.
• Familiarity with therapy techniques and methodologies relevant to elderly populations.
• Demonstrated ability to advocate for culturally competent care.
