Job description

If you are someone who enjoys offering a creative, energetic, and caring environment to individuals with barriers to independence, you may be interested in a Supported Community Living Associate position with Goodwill of the Heartland. 
Our Supported Community Living Associate is responsible for developing and maintaining relationships with program participants, as well as finding and utilizing community resources that may benefit our program participants. This is a role that helps to identify the treatment and service needs of each participant in our program, as well as monitoring and evaluating the progress of the treatment plan, assisting and facilitating as appropriate. This position helps to manage expenses, bill hours, and write high quality case notes. They assist the Supported Community Living Specialist with service plan developments and progress reports.  
Essential Duties and Responsibilities:

  • Develop and maintain effective relationships with clients. 
  • Identify and maintain current information about community resources that may benefit program participants. 
  • Identify treatment and service needs of each individual served. 
  • Monitor and evaluate progress of the treatment plan, assisting and facilitating as appropriate. 
  • Monitor and control expenses and ensure efficiency in rehabilitation services, including billable hours and client funding. 
  • Write quality case notes and assist SCL Specialist with ISP Development and Progress Reports. 
  • Provide transportation to participants as needed. 
  • Have access to a safe, reliable vehicle.

Qualifications:  

  • A high school diploma or GED required 
  • Six months experience in training and/or rehabilitation required. Post-secondary education may be substituted for experience requirement.  
  • Reliable vehicle, valid Iowa driver’s license, proof of auto-insurance, and successful completion of a motor vehicle record screening, dependent and adult child abuse, and criminal background check 

 
The schedule for our Supported Community Living Associate is Monday – Friday, 8:00am – 5:00pm, 40 hours a week. Travel between participant locations is required. The starting wage for the position is $16.78.
