Home Health Nurse Liaison (RN)

Home Health Registered Nurse Overview:
  • Competitive Compensation:
    • Full-Time Employees*: $90,000 - $110,000 (+ mileage)
  • Collaborate with the patient, patient's family, the acute care hospital or sub-acute facility, physician group and/or health staff to facilitate a safe and effective transition to home care

Your Role and Responsibilities as a Home Health Registered Nurse:
  • Assess the patient's and family caregiver's homecare needs and coordinates home health care services as required prior to the patient's transition home
  • Acts as clinical resource to coordinate complex cases for safe and appropriate transition to the patient's home
  • Examine patients, conduct assessments, and document patient progress while effectively communicating with other healthcare providers
  • Educate patients and their families on care plans, medications, therapies, diet, and exercise
  • Complete and present patient reports to facilities and management teams
  • Works collaboratively with Office Intake Department in verification of coverage or payment
Qualifications for an Interim HealthCare® Home Health Registered Nurse:
  • Graduate of an accredited Nursing Program with an active RN license in IL
  • Minimum 2 years of nursing experience, ideally in home healthcare
  • Current CPR and BLS certifications
  • Knowledge of state and federal home health regulations
  • Strong clinical judgment, excellent communication skills, and a compassionate approach
