About the Role:Job Summary

The Cognitive Nurse Navigator, a Registered Nurse, is responsible for the navigation of the patient through the continuum of healthcare during the patients time within the NGPG practice, and during the time in which they are under the care of an NGPG Neurologist for a cognitive disorder. Significant implementation includes planning, implementation, coordination, monitoring, and evaluation of individualized care options, services, resourcing, education and follow up needs.

Minimum Job Qualifications
  • Licensure or other certifications: Licensed to practice as a RN in Georgia required.

  • Educational Requirements: Bachelors Degree in Nursing.

  • Minimum Experience: Must have a minimum of five years of patient care experience.

  • Other: Proof of Professional organization membership within one month from date of hire required.

Preferred Job Qualifications
  • Preferred Licensure or other certifications:

  • Preferred Educational Requirements: Masters Degree (Nursing or other if possessing a BSN).

  • Preferred Experience: Neurologic nursing care and/or case management experience

  • Strong organizational skills and the ability to work independently with minimal supervision

  • Strong nursing skills with extensive nursing knowledge and ability to safely triage patient concerns and make appropriate recommendations or escalate appropriately for the cognitive patient population

  • Analytical ability necessary to plan and coordinate the cognitive continuum of care along side the physician to ensure its effectiveness

Essential Tasks and Responsibilities
  • Collaboration and professional communication with the outpatient neurology team to provide seamless care to the patient during the episode of care.

  • Educates and collaborates with neurology staff, physicians, and the community as well as patients and their caregivers regarding services.

  • Using Evidenced-Based Practice, implements chronic care management plan to assure optimal quality of life outcomes.

  • Assists patient/family in understanding their diagnosis, treatment options and resources available.

  • Assists with the education of staff members utilizing best practice standards, order sets in consensus with national guidelines.

  • Functions as an expert Clinician and mentor for staff.

  • Assists in tracking data, outcomes, and supporting strategic and performance improvement activities.

  • Practices LEAN principles to streamline care across the continuum, being mindful of cost and quality outcomes.

  • Leads education groups and monitors outcomes, e.g. patient satisfaction, compliance with treatment regimen.

  • Must have critical thinking skills, be able to problem solve independently, work well in a team environment. Strong written and verbal communication skills.
