JOB LOCATION : Hospice
DEPARTMENT: AR - HOSPICE
SCHEDULE: PRN, 8 HR Day Shift, 8am-4:30pm M-F
POSITION SUMMARY Responsible as a member of the interdisciplinary team for assigned functions related to patient admissions. Collaborates with the Hospice Medical Director to help ensure eligibility and appropriateness for admissions to the hospice program. Performs initial patient assessments and assists in the development and implementation of the initial Plan of Care. Ensures continuity of care throughout the admissions process. Provides education on hospice care and services as needed. Supports other aspects of Hospice nursing services as census/patient need requires. Rotates weekend on-call rotation upon request. KNOWLEDGE, SKILLS & ABILITIES Required current licensure as a professional Registered Nurse in the state of Georgia, with BSN or higher professional nursing degree preferred. Required minimum of two (2) years varied work experience as a Registered Nurse (prefer prior experience in hospice/palliative care)Demonstrated understanding of and adherence to the hospice philosophy and ability to work as member of interdisciplinary team in meeting the unique needs of the terminally ill. Certification in hospice/palliative care strongly preferred. Must demonstrate attention to detail, knowledge of diagnosis codes, ICD-10 experience preferred. Must possess strong clinical skills in assessment, planning, intervention, and evaluation of patient care. Must have positive attitude with demonstrated ability to handle delicate situations and to communicate effectively with members of the hospice team, physicians and other referral sources, patients/families, and community agencies/institutions. Must possess ability to work non-judgmentally with persons of diverse cultural, religious, and socioeconomic backgrounds and lifestyles. Must be self motivated with strong organizational skills and demonstrated ability to utilize critical thinking techniques and problem-solving techniques with minimal direction and appropriate collaboration. Ability to operate clinical equipment, and office equipment such as computers, telecommunications devices, etc. Must have current GA driver's license and satisfactory driving record. Must have current CPR certification
