Job description

Hospice Certified Nursing Assistant

Salary: $19-22/hr BOE

Schedule: Monday-Friday 7 AM- 3:30 PM

Location: Kettering, Ohio (surrounding area)

Requirements of the CNA role:

  • Licensed Nursing Assistant in the state of practice (CNA)
  • BLS Certified
  • Prior experience working in an AL/LTC/SNF facility or Home Health REQUIRED
  • Enjoy working with Senior Citizens, Positive Attitude, Professional, Responsible & Dependable
  • Must be willing to TRAVEL
  • Valid driver's license and Auto insurance Coverage with reliable transportation
  • Hospice experience preferred

About the Certified Nursing Assistant role:

  • Provide compassionate, patient-centered personal care including bathing, grooming, dressing, toileting, and feeding in accordance with the hospice aide plan of care.
  • Travel to patientsโ€™ residences, including private homes and facility settings, to deliver hospice services in a respectful and professional manner. Offer emotional support and companionship to patients and families, including spending quality companionship time to promote comfort, dignity, and quality of life.
  • Provide bedside vigil support as needed during end-of-life care, ensuring patient comfort and presence during critical moments.
  • Assist patients with ambulation, transfers, positioning, and range of motion exercises as documented in the hospice aide plan of care.
  • Safely use and assist patients with durable medical equipment (DME) such as walkers, wheelchairs, hospital beds, and lifts.
  • Observe and report changes in the patientโ€™s physical condition, emotional status, or home environment to the case manager or supervising nurse.
  • Accurately document care provided in accordance with Care 360 Hospice policies and procedures.
  • Foster a cooperative team environment by understanding the roles of interdisciplinary team members and actively participating in team meetings and staff conferences.
  • Provide support and education to family members and caregivers within the scope of the CNA role.
  • Participate in required on-site monthly educational meetings and ongoing training to maintain competency and compliance.
  • All other duties as assigned
