Department: Stroke & Neuroscience
Location: Forsyth Medical Center - Winston Salem, NC
Schedule: Full-Time | Monday-Friday, 8:00 AM - 4:00 PM
Flexibility required for occasional early mornings or late evenings
Position Overview:
The Stroke Coordinator plays a vital role in advancing high-quality stroke care by partnering with the interdisciplinary healthcare team across all aspects of the patient journey. This position leads both formal and informal education for staff, supports patient and family education, and drives performance through the collection and analysis of clinical, financial, and quality data. The ideal candidate is passionate about improving outcomes, fostering collaboration, and enhancing stroke program excellence.

What You'll Do
The Coordinator collaborates with the health team in all aspects of patient care. Provides formal and informal education for staff and facilitates patient education. Collaborates with nursing staff to facilitate patient care and with health management regarding appropriate discharge options, across the region. Collects and manages clinical, financial and quality data for stroke.
  • Coordinates and oversees compliance with regulatory standards to maintain primary stroke center certification.
  • Leads program evaluation and assists with process improvement efforts through established committees and meetings, such as Stroke council meeting and neuro best practice exchange team.
  • Services as the liaison between physicians, managers, and clinical staff of Novant Health Forsyth Medical Center and Novant Health stroke and Neurosciences institute
  • Promotes stroke care by creating collaborative relationships between healthcare facilities in the Forsyth Medical Center region.
  • Reviews and evaluates data monitoring and analysis to facilitate meeting program objectives and improvements in quality/patient outcomes
  • Coordinates, develops, participates in, and/or facilitates education surrounding stroke and neurologic emergencies for hospital and EMS personnel.
  • Closely works with stroke manager and community outreach to promote stroke education in the region.

What We're Looking For
Minimum of 3 years of RN experience
  • Education: 4 Year / Bachelors Degree, required. BSN, required.
  • Experience: Minimum of three years of clinical RN experience, required.
  • Licensure/certification/registration: Registered as RN in NC or compact state, required. Certification as CNRN or SNRN, preferred.
  • Additional skills required: Demonstrates interdisciplinary leadership skills, superior interpersonal communication skills and knowledge of computer database, data analysis and presentation skills.
