Job description

JOB DESCRIPTION: Clinic Nurse (RN or LPN)

REPORTS TO: Site Administrator

DEFINITION OF POSITION: The RN or LPN provides individualized and specialized nursing care to patients in the Clinic in accordance with the physician's orders, recognized nursing techniques, legal aspects of the current nurse practice act within the state of Idaho, and the Practice's policies and procedures. The registered nurse or licensed practical nurse functions cooperatively with physicians and provides support to the clinic's medical assistant resource team, exhibiting teamwork characteristics. Demonstrates initiative, insight, and judgment in the decision-making process.


GENERAL RESPONSIBILITIES:
1. Accept directions from supervisors, physicians, and advanced practice providers and adhere to
established policy and procedures.
2. Consult with appropriate supervisors, physicians, and advanced practice providers as needed.
3. Maintain the patient's right to privacy by not discussing privileged information outside the work area or within hearing range of visitors/family or patient.
4. Function within the legal limits of the Nurse Practice Act.
5. Refer situations beyond the ability or function to the proper person.
6. Seeks educational opportunities and accepts responsibility for own professional growth and development. Participate in in-service opportunities and staff meetings.
7. Partners with providers associated with the designated site, yet is assistive to all providers at IGA when needed.
8. Maintains computer skills necessary to function in the electronic medical record and Idaho Health Data Exchange.
9. Supports physicians, advanced practitioners, and medical assistants in high-level direct patient care for high-risk patients, including inpatients transitioning to outpatient care, IBD to include infusion therapy, Cirrhotic requiring diuretic management, pancreato-biliary disease, transplant patients, newly diagnosed colon, liver, or esophageal cancer, gastroparesis, pancreatitis, and PEG patients
10. Facilitate patient access to services provided at IGA. Additionally, facilitate communication between IGA providers for the transition of care via referral.
11. Investigate and refer patients to appropriate social service programs.
12. Follows current prescription policy.
13. Assists in keeping staff break room neat.
14. Utilizes Microsoft Outlook email for non-patient correspondence and the electronic medical records messages for patient care correspondence/documentation.
15. Demonstrates respect for IGA policies for security, privacy, and confidentiality while using IGA equipment
16. Immediately report any network, hardware, or software malfunction to the IT Officer or Supervisors
17. Communicates pertinent information to others.
18. Perform related and other duties as assigned by supervisor
19. Maintain compliance with all company policies and procedures.

SPECIFIC POSITION TASKS:
1. Maintains a highly functioning relationship with the Medical Assistant to provide support when the medical assistant is uncertain of the next steps or the patients' needs are beyond the Medical Assistant's scope of practice as defined in the Medical Assistant job description. The Registered Nurse will give directions that are within the medical assistant's scope of practice.
2. Accepts incoming calls from patients, physicians, and other staff collaborating in the care of the patient. Priorities call for the most urgent matters to be handled first.
3. Provides patient follow-up for complex care management of new cancer diagnosis and highly complex disease management.
4. Monitors and documents patients' condition and effectiveness, and the timeliness of care and/or interventions.
5. Note and carry out the physician's orders.
6. Coordinates patients' care plan and educational needs. Provides educational material as appropriate.
7. Assist physicians with complex lab result messaging upon request.
8. The clinic RN will actively be involved in nurse visits for patient education for specific diagnoses such as inflammatory bowel disease and medications to treat hepatitis C, Lovenox bridge education, diabetic medication instructions for patients having procedures, high-risk medications to include diuretics, and education on self-injection. Obtains informed consent for medications when providing education.
9. As a part of the initial education for chronic disease management, the Clinic RN will begin the documentation process for chronic care management. This includes a detailed note on the education visit, confirming the next steps in the care plan, documentation of the time spent in education, and follow-up as a chart note. This also includes billing the internal tracking code CCM05 for each 5-minute increment of time spent.
10. Schedule patients for office visits if identified that an appointment is necessary during a phone call. Enter recall appointments as needed.
11. Works with physicians to document medication management protocols for the MAs to follow. Provides direct patient support when patient circumstances require deviation from standard protocols.
12. The RN collaborates with the triage nurse team, the prior authorization team, and other department staff within IGA and Endoscopy to enhance patient care and outcomes.
13. Communication to tertiary care centers as needed for patient follow-up and new patient referral to IGA.

PHYSICAL REQUIREMENTS:
Full Range of body motion, including handling and lifting patients
Requires working under stressful conditions and requires standing and walking for extended periods of time.
Exposure to communicable diseases and/or bodily fluids
Ability to safely and successfully perform the essential job functions consistent with the ADA, FMLA, and other federal, state, and local standards, including meeting qualitative and/or quantitative productivity standards.
Ability to maintain regular, punctual attendance consistent with the ADA, FMLA, and other federal, state, and local standards.
Must be able to lift and carry up to 50 lb.

QUALIFICATIONS:
Associate's degree in nursing
Two (2) years of nursing experience preferred.
One (1) year of experience in a comparable setting preferred
Valid nursing license in Idaho.
