Job Summary
Program Manager, RN Southeast/PearlandPosition will participate in multi-disciplinary rounds, facilitate patient satisfaction escalations, review quality metrics, conduct quality audits, monitor dashboards, play an active role in case management discharge planning and participate in continuous quality improvement with external referral sources, social workers, discharge planners, case managers, healthcare professionals, families and patients.Job Description
Minimum Qualifications
Education:Graduate of an accredited School of Professional Nursing, Bachelor's of Nursing, required
Licenses/Certifications:
Certified in Basic Life Support
Current State of Texas license or temporary/compact license to practice professional nursing
Experience / Knowledge / Skills :
Minimum of five (5) years nursing experience in an acute care setting
Two (2) years of previous supervisory experience required
Additional previous case management, quality and/or clinical documentation knowledge is preferred
Understanding of quality improvement, performance improvement, change management and patient safety knowledge preferred
Working knowledge of Cerner Electronic Health Record preferred
Proficient in Microsoft Office, including Excel, Word, PowerPoint and Visio
Clinical knowledge of assigned clinical conditions and physiological systems to discern the impact of specific indicators on outcomes and to identify potential gaps in clinical care processes
Ability to interact with all members of the healthcare team and administrative staff in a professional and courteous manner
Interpersonal skills necessary in order to interact effectively with external referral sources, including social workers, discharge planners, case managers, other healthcare professionals, families and patients
Demonstrate strong skills and a track record for sustained measurement of program outcomes, ability to work independently and with and through others to achieved desired results
Ability to build relationships and gain alignment for clinical support from superiors and peers
Ability to multi-task, prioritize work, pay close attention to details and meet deadlines
Excellent organizational and communication skills
Ability to effectively communicate complex ideas (both conceptual and concrete) by oral and written means

Principal Accountabilities
Leads and coordinates onboarding for Physicians and Advanced Practice Providers (APPs), ensuring all onboarding logistics are executed effectively for both clinical and non-clinical team members.
Oversees Business Coordinator functions, ensuring operational efficiency and accountability.
Oversees all program billing processes to ensure accuracy and compliance.
Partners with the Physician Lead to develop and maintain Physician and APP schedules to ensure appropriate staffing coverage and operational continuity.
Collaborates with, directs, and may delegate to the healthcare team in the attainment of positive outcomes based on data, research, evidence, and professional standards of care.
In conjunction with the Medical Director, oversees clinical workflow of the program to ensure quality of care delivered and patient satisfaction is optimal.
Serves as super user for EMR.
Participates in a systemic, interdisciplinary, and ongoing evaluation of programs, process improvement and desired client-centered outcomes.
Leads in-services, educational programs, required department meetings, and continuing education courses to keep team abreast of new developments.
Develops strong working relationships with external referral sources and internal staff including Admissions, physicians, Outpatient Clinic, Marketing staff and other departments; and ability to adhere to confidentiality policy and procedures. Connect the hospitalists' plan with primary care providers, chronic disease care clinics, insurers, specialists, and post-acute providers when necessary.
Implements and evaluates changes in clinical and operational processes.
Possible coverage of additional campuses.
Ensures safe care to patients, staff and visitors; adheres to all Memorial Hermann policies, procedures, and standards within budgetary specifications including time management, supply management, productivity and quality of service.
Promotes individual professional growth and development by meeting requirements for mandatory/continuing education and skills competency; supports department-based goals which contribute to the success of the organization; serves as preceptor, mentor and resource to less experienced staff.

The Memorial Hermann Southwest Hospital Women's Services is a magnet hospital as well as a level 3 designated facility with a blend of high-risk and community patients. This creates a fast-paced environment and a chance to work with a diverse population. Our Labor & Delivery unit has a low c-section rate and is extremely collaborative and close-knit.
