Overview
Pleasant Vine Home Care Delaware is seeking a dedicated, committed, and organized Home Care Manager/Care Coordinator to oversee the daily operations of our home care agency. The Care Manager/Coordinator plays a vital role in ensuring the quality and continuity of care by overseeing client intake, coordinating schedules and staffing, and managing daily home care operations. This multifaceted position requires excellent organizational, communication, and problem-solving skills to ensure that all client needs are met in a timely and efficient manner.

Key Responsibilities

Client Intake & Coordination

  • Answer phone calls and respond to client inquiries in a timely, professional manner.
  • Perform initial intake of new clients, including assessments and service needs.
  • Maintain accurate and up-to-date client records in the agency management system.
  • Coordinate the start-of-care process with clinical and administrative teams.
  • Verify client insurance coverage and/or payer source eligibility.

Scheduling & Staffing

  • Schedule caregivers based on client needs and staff availability.
  • Maintain accurate schedules and proactively update changes.
  • Ensure proper coverage for all shifts, including emergencies and call-outs.
  • Match caregivers with clients based on skills, experience, and compatibility.
  • Communicate assignments clearly to staff and confirm schedules.
  • Monitor daily clock-in/out compliance and resolve discrepancies.

Staff Management & Supervision

  • Assist with onboarding, orientation, and training of new field staff.
  • Maintain accurate employee files and credentials (licenses, TB, CPR, etc.).
  • Track caregiver compliance with state and agency requirements.
  • Communicate performance issues to management and assist in resolution.
  • Support team development and foster a positive work environment.

Operational & Administrative Tasks

  • Use home care software to manage scheduling and documentation.
  • Prepare reports on staffing, client care, and visit logs as needed.
  • Ensure compliance with HIPAA and other regulatory standards.
  • Participate in audits and ensure records are accurate and compliant.
  • Support payroll and billing by confirming time logs and visit completion.

Customer Service & Communication

  • Build and maintain positive relationships with clients, families, and referral sources.
  • Address client complaints and concerns professionally and promptly.
  • Serve as the liaison between clients, families, and field staff.
  • Participate in care plan meetings and service adjustments as needed.

Qualifications

  • High school diploma or GED required; associate or bachelor’s degree preferred.
  • Minimum 1–2 years of experience in home care, healthcare, or scheduling preferred.
  • Experience with home care software and EMRs is a strong plus.
  • Excellent communication, interpersonal, and organizational skills.
  • Ability to multitask, prioritize, and problem-solve in a fast-paced environment.
  • Knowledge of home care regulations and caregiver credentialing.
  • Bilingual preferred (Spanish, Creole, etc.), depending on client population.

Work Schedule

  • Monday to Friday, 9:00 AM – 5:00 PM (on-call rotation or weekend availability as needed)
