Job description

Excelsior Wellness is seeking an employee to serve as the HUB Access Coordinator, an additional leadership assignment that supports referral operations, client access, and successful connections to services across the organization.
This is not a standalone position. The selected employee will maintain their current role while dedicating approximately 15-20 hours per week to HUB Access Coordinator responsibilities. Time allocation may vary based on referral volume, staffing coverage, and operational needs.

The HUB Access Coordinator helps ensure every individual expressing interest in services is welcomed, contacted, guided, and connected to the appropriate program. By supporting referral workflow from initial interest through enrollment, this role strengthens access to care, improves communication, reduces delays, and supports a consistent experience for individuals, families, referral sources, and program teams.
Position Summary:
The HUB Access Coordinator provides day-to-day operational leadership for referral, outreach, intake coordination, client navigation, and enrollment support activities. The role works closely with Care Coordinators, Community Health Workers, and other designated staff responsible for receiving, assigning, tracking, and following up on referrals.
The coordinator reports to and works in partnership with the Executive Director of EFM, who oversees HUB referral operations, and communicates referral trends, operational barriers, staffing needs, and performance outcomes.
Key Responsibilities:
Team Leadership, Staff Support, and Workflow Oversight
  • Provide day-to-day guidance, coaching, onboarding support, workflow oversight, and staffing coverage coordination for referral and intake-related activities.
  • Support Care Coordinators, Community Health Workers, and other designated staff in managing referral workflows and follow-up expectations.
  • Foster a positive, strengths-based team culture focused on collaboration, accountability, customer service, and consistent referral management practices.

Referral Management, Communication, and Enrollment Coordination
  • Oversee referrals from initial interest through enrollment to ensure timely review, assignment, documentation, outreach, and follow-up.
  • Monitor referral volume, timeliness, outreach activity, enrollment outcomes, and other key performance indicators.
  • Ensure individuals, families, and referral sources receive clear and consistent communication regarding referral status, next steps, and enrollment readiness.
  • Identify and address barriers that may delay referral completion, intake scheduling, enrollment, or access to services.

Quality, Process Improvement, and Documentation
  • Ensure accurate and timely documentation, record management, tracking, and enrollment updates within the electronic health record.
  • Monitor referral and enrollment workflows to ensure compliance with organizational policies, documentation standards, privacy expectations, and established procedures.
  • Recommend and support process improvements that enhance efficiency, communication, and access to services.

Qualifications:
  • Three years of leadership, supervisory, or team coordination experience in healthcare, behavioral health, social services, care coordination, referrals, intake, or a related field.
  • Strong understanding of customer service, referral management, person-centered engagement, and client navigation.
  • Excellent communication, organization, problem-solving, relationship-building, and prioritization skills.
  • Ability to effectively coordinate multiple priorities in a fast-paced environment.

Additional Assignment Information:
The HUB Access Coordinator is an organizationally designated leadership assignment and is not a separate position classification. Employees selected for this opportunity will continue to serve in their primary role while fulfilling HUB Access Coordinator responsibilities as assigned.
Additional Compensation:
The selected candidate will get a $425 monthly stipend.
