Job description

MUST HAVE ACTIVE BLS & WI LICENSE AT TIME OF SUBMISSION. Shift: 8hour PM shift, (40 hours/week) Shift Information: 2pm - 10p Weekend Rotation: Every other weekend required Holiday requirements: Every other holiday required Ratio: 1: 8-10 residents Must Have: 2+ years of experience Current certification as a Certified Nursing Assistant on the Wisconsin Nurse Aide Registry • Ability to lift 50+ lbs and support residents safely. Computer literacy, which includes electronic medical records.

Essential Job Duties & Responsibilities: Care, feeding, and mobility support. Follow individualized care plans and ensure care is delivered in alignment with resident goals, preferences, rights, and safety needs. • Assist with ambulation, transfers, and positioning using gait belts, non-mechanical aids, and mechanical lifts (sit-to-stand, full-body lifts).

• Provide restorative and range-of-motion exercises as assigned. • Answer call lights promptly and ensure timely, compassionate response to resident needs. • Assist residents with assistive devices, prosthetics, colostomy appliances, and adaptive equipment.

• Support residents during mealtimes, including set-up, per-meal grooming, feeding assistance, documentation of intake, and ensuring nutritional safety. • Observe and report changes in resident physical, emotional, or cognitive status to the Licensed Nurse. Communicate effectively with residents, families, nurses, and interdisciplinary staff while maintaining professionalism and empathy.

• Provide input to care conferences and document care accurately in electronic systems. • Maintain a clean, safe, and organized resident environment including stocking supplies, making beds, and assisting with resident personal laundry. • Follow infection control protocols, including PPE use, hand hygiene, standard and transmission-based precautions.

• Report equipment concerns, safety hazards, or resident rights issues immediately. • Support a calm, homelike atmosphere and assist in minimizing stressors for residents. Protect resident belongings and promptly report missing items.

• Ensure residents are free from abuse, neglect, and restraints. • Honor the residents' right to refuse care and seek alternatives that respect autonomy. • Accurately document ADLs, intake/output, restorative care, and other assigned tasks in the electronic charting system prior to end of shift.

• Participate in onboarding and training of new staff members. Maintain awareness of unit-specific needs, including ventilator care, dementia care, and end-of-life/hospice support. • Participate in emergency procedures, including identifying Dr.
