WHAT DO WE DO?

CHSI is a private, non-profit organization established in 1973 to serve the health needs of migrant & seasonal farm workers and their family members. Today, CHSI has expanded its services and provides mental & physical healthcare to all individuals in our communities. Our services are provided on a more affordable sliding fee scale to those who are underinsured or uninsured. We also accept ND and MN Medicaid, Medicare, and most private insurance plans. We currently operate four nurse-managed clinics, and two victim advocacy program centers.

WHO ARE WE LOOKING FOR?

We are searching for a Per diem Mobile Health Provider (MD/DO) to join our Rochester, MN team. This is a per diem position with a flexible schedule based on mobile health operations and community need. Work is performed exclusively in mobile unit and community-based settings. Hours may include daytime mobile clinic coverage, outreach activities, and occasional evening, after-hours, and weekend events as needed.

Duties:

  • Provides primary care services to patients on CHSI mobile health units and outreach sites.
  • May assist the Medical Director in the revision and implementation of practice principles, clinical protocols, and health care plans as needed.
  • Consults with staff nurse practitioners and physician assistants as directed.
  • Follows all applicable national patient safety goals in the provision of patient care.
  • Serves as a role model for effective teamwork among staff and actively works to maintain positive morale and attitude. Leads the team in the team-based care model.
  • Facilitates coordination and follow-up care between mobile health services and in-clinic providers to support continuity of patient care as needed.
  • Provides care in collaboration with the center's team of physicians, nurse practitioners, behavioral health providers, nurses, and medical assistants.
  • Serves as a resource person for clinical staff regarding patient care issues.
  • Establishes and maintains rapport with community agencies.
  • Perform duties are essential to the position and program.
  • Demonstrates sincere dedication and loyalty to the philosophy and mission of CHSI.

ARE YOU A GOOD FIT?

In addition to being a wonderful team member, we are seeking an individual with the below qualifications:

PHYSICAL ABILITIES

  • In an eight hour workday, the employee must be able to: bend and crouch occasionally, sit continuously and stand or walk frequently.
  • Employee must be able to lift up to 50 pounds occasionally.
  • Employee must be able to use hands/arms, feet/legs for repetitive fine to large motor skill movements.

Qualifications:

  • Graduate of an accredited Doctor of Medicine or Doctor of Osteopathic Medicine program.
  • Minimum of 2 years of experience in community health or primary care setting

Knowledge/Skills/Abilities:

  • Ability to provide effective care consistent with medical best practices.
  • Ability to see all ages of patients, including pediatrics and geriatrics.
  • Ability to provide prenatal care in an outpatient setting.
  • Ability to perform women's health procedures including LARC placement, Endometrial biopsy, and possibly colposcopy.
  • Ability to provide healthcare in an unbiased manner to patients of many cultural backgrounds.
  • Ability to work independently.

Licensure and Credentials:

  • Licensure and certification as required by the discipline.
  • MN OR ND state license
  • DEA and controlled substance license
  • BLS Certification
