Description

Health Solutions is a premier wellness center focused on whole-person care. With over 400 employees in Southern Colorado, you would be joining a mighty team of support and administrative staff, clinicians, physicians, nurses, and others in our efforts to improve the health and wellbeing of our community.

We're hiring a CIT Co-Responder Clinician to join us at our Rural Health Solutions location in collaboration with Trinidad Police Department. Must be available to work Tuesday-Friday from 7:00 AM-5:00 PM, with the flexibility to adjust the schedule as needed to meet the needs of the program.

As a CIT-Co-Responder Clinician, you would provide early intervention assessment, referrals and clinical de-escalation services to persons in the community that are identified by Trinidad Police Officers as having behavioral health related concerns. To provide information, education, liaison, and transition services to various networks within Health Solutions and with federal, state, and community-based agencies and resources. To meet the contractual obligations of a sole source contract with the Trinidad Police Department. To provide training and on-going education for officers to increase awareness of crisis de-escalation, mental health and substance use issues and intervention. To provide family members with necessary information and education to further assist persons with mental illness and substance use disorders and for themselves.

Specifically, the CIT-Co-Responder Clinician under the administrative supervision of the Program Director, and the clinical supervision of the Program Director, is responsible for:

  • Accompany Trinidad Police Department officers on mental health related calls in the community, triage, provide crisis intervention, assess needs and provide resources, link with treatment
  • Assessment of all referrals to determine appropriateness for services at Health Solutions
  • Provide case management services including linking to services, monitoring, follow-up, and crisis management for clients
  • Provide services to meet need: office based, community based, or in the client's home
  • Make appropriate referrals to and act as liaison with community agencies, service providers, and natural support systems
  • Provide in-reach and out-reach at designated facilities as assigned/appropriate
  • Collaboration with client(s), family members, and significant others in identifying strengths and treatment needs
  • Work with client and family/support system for appropriate service delivery
  • Provide assistance in developing community-based resources for social, vocational, and leisure skills
  • Maintain communication and links with other Health Solutions programs e.g. Crisis Services, Outpatient, Case Management, Vocational, and Recovery Services programs
  • Coordination for emergency services when clinically indicated/medically necessary; risk/lethality assessment (including inpatient admission)
  • Resource acquisition and assistance with application for benefits and entitlements in collaboration with Case Management Team
  • Assist clients with basic needs such as housing, food, clothing, medical and dental care, legal, vocational, and psychiatric services
  • Provide on-going training and educational opportunities for officers to increase awareness of de-escalation, mental health and substance use intervention
  • Provide family members with necessary information and education to further assist clients and themselves
  • Participate in team meetings, clinical staffing's, and clinical consultation as scheduled
  • Utilize clinical practice guidelines, Health Solutions and State Division of Mental Health standards for appropriate service delivery
  • Appropriately screen and assess referrals for admission into the Crestone Recovery program or other Health Solutions Networks; document, process and organize referrals in a timely manner
  • Coordinate consumer needs and fosters relationships while working with Trinidad Police Department and other criminal justice representatives (jail, courts, Parole Probation etcโ€ฆ)
  • Manage and maintain accurate and timely data collection and documentation database
  • Work with management team on reports, presentations as assigned

Qualifications

What We're Looking Forโ€”The Must-Haves

  • Master's degree in social work, counseling, psychology, or a closely related field will be considered contingent upon eligibility for license within a specified time frame.
  • Experience working with SUD and Co-Occurring disorders
  • Experience conducting brief therapy, working with different age groups, and individuals with acute behavioral health issues.
  • Computer proficiency
  • Valid Colorado driver's license, and access to transportation during work hours
  • Must be able to work Monday-Friday from 8:00 AM-5:00 PM.

What We'd Like to See in Youโ€”The Nice-to-Haves

  • Colorado clinical license (i.e., LPC, LCSW, LMFT)
  • Colorado Certified Addiction Specialist or Licensed Addiction Counselor.
  • Experience working with a multi-disciplinary team.
  • Flexibility, and exceptional time management and documentation skills.
  • Spanish language skills
