About the Role
As a Remote RN Care Manager, you'll play a key role in managing patients enrolled in our RPM/CCM programs.
You'll conduct virtual assessments, monitor clinical data from remote devices, provide ongoing patient education, and collaborate closely with providers to identify changes in condition and adjust care plans. This role is ideal for an RN who enjoys autonomy, critical thinking, and building long-term patient relationships-without bedside burnout.
What You'll Do
  • Conduct virtual patient assessments and individualized care management sessions
  • Monitor and interpret data from remote patient monitoring devices
  • Manage and support patients with chronic conditions, primarily CHF
  • Deliver patient-centered education focused on:
    • Disease management
    • Medication adherence
    • Symptom recognition and escalation
  • Develop, reinforce, and adapt care plans in collaboration with providers
  • Identify clinical trends or changes in patient condition and escalate appropriately
  • Guide patients in sustainable lifestyle modifications including:
    • Nutrition and dietary changes
    • Activity tracking
    • Stress and symptom management
  • Document and communicate clinical findings clearly within the care team
  • Serve as a trusted clinical resource for patients navigating chronic illness
What We're Looking For
  • Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensed in both New York and Hawaii
  • 2+ years of clinical nursing experience
  • Prior experience in RPM, CCM, telehealth, or care management strongly preferred
  • Bilingual English/Spanish is a plus (not required)
  • Comfortable working independently in a virtual environment
  • Strong clinical judgment, communication skills, and tech proficiency
