Job description

About 9amHealth
9amHealth is an AI-enabled virtual specialty care platform focused on managing high-cost chronic conditions at scale. The company partners with employers, health plans, and pharmacy benefit managers to deliver comprehensive, cost-effective medical care for individuals living with obesity, diabetes, hypertension, and dyslipidemia. Members receive access to specialized clinicians, including endocrinologists, obesity medicine specialists, and clinical pharmacists, at-home lab testing, prescription medications, and lifestyle support.
9amHealth was founded in 2021 and is backed by leading healthcare investors like Define Ventures, SemperVirens, 7Wire Ventures, and The Cigna Group Ventures.
What it is
We are looking for an RN to deliver patient centered health coaching and chronic disease management via chat, phone, and video. This role includes triaging symptoms, providing education on diabetes, hypertension, hyperlipidemia, and weight management, coordinating care with providers and cross functional teams, and documenting patient care accurately in a telehealth setting.
For you, caring for people with chronic conditions like diabetes and obesity isn't a job, it's a calling. You live and breathe it. You feel a deep desire to be a force of positive change, for the individuals you care for, but also within the field of cardiometabolic health at large. You speak compassionately but earnestly, committing yourself fully to helping people understand their conditions, but also have faith in what's possible with the right support and resources.
You act from a place of empathy, always: taking the time to ask tailored questions, and get into the weeds with people to find the right solutions for them. In turn, the people you care for see you as a confidante. They turn to you because they know you'll listen, and because they know that when they ask for help, you'll go the extra mile to help them thrive.
Our Registered Nurse Clinical Care Specialists provide direct care and education to patients with chronic cardiometabolic conditions, working closely with our Providers.
What you'll do
  • Deliver patient centric health coaching that integrates medical needs with patient preferences.
  • Triage patient reported symptoms and side effects according to internal protocols, escalating to Providers when appropriate.
  • Provide comprehensive diabetes, hypertension, hyperlipidemia, and weight management care and education via chat, phone, and video.
  • Coordinate care seamlessly between patients, providers, support teams, and cross-functional clinical team members.
  • Create consults that align with 9amHealth's internal protocols for Providers.
  • Document patient care accurately and consistently to meet internal standards.

What you'll need
Qualifications
  • Strong empathy, communication, and patient-centered care skills
  • Bachelor of Science in Nursing (BSN) required
  • Active Registered Nurse license in good standing, with ability to obtain additional licensure if needed
  • Certified Diabetes Care and Education Specialist (CDCES) or willing to pursue if not currently certified
  • 3+ years of direct patient care in an ambulatory or telehealth setting
  • Adaptable, self directed, and comfortable working independently
  • Proficiency with digital tools for care delivery
  • Flexibility to participate in rotational weekend or holiday coverage
  • A HIPAA compliant, distraction free workspace

This role offers a salary of up to $100,000, depending on experience.

We offer a comprehensive benefits package, including health, dental, and vision insurance, along with flexible PTO and work from home options. We provide a professional development budget and support continuing education, empowering our team to grow alongside the company. Team members have the opportunity to make a meaningful impact on chronic disease management and patient outcomes while contributing to a collaborative, mission driven culture focused on improving healthcare accessibility and affordability.

We are committed to creating a workforce that reflects our patients and creating a positive healthcare experience for them. We welcome applications from people of color, LGBTQ+ individuals, people with disabilities, members of ethnic minorities, and foreign born residents.
