Position Summary

The Case Manager / Client Care Coordinator is responsible for coordinating client care services both within the clinic and throughout the community. This role provides individualized support through needs assessments, goal planning, resource coordination, mentoring support, and follow-up care for clients before and after childbirth.

The Case Manager serves as an advocate, encourager, connector, and trusted support person for clients while helping them move toward stability, growth, and long-term wellbeing.



What You'll Do


Client Care & Case Management

  • Conduct comprehensive client assessments including physical, emotional, social, educational, financial, and support-system needs
  • Develop individualized Client Care Plans with measurable goals and action steps
  • Support clients through ongoing encouragement, accountability, and resource coordination
  • Assist clients in accessing community programs, government assistance, and support services
  • Track client progress and document outcomes accurately within Salesforce
  • Provide crisis prevention and intervention support when necessary
  • Perform due-date follow-ups and occasional home or hospital visits for mothers and newborns
  • Support clients with emergency assistance needs including diapers, baby items, clothing, and other practical resources
  • Follow up regarding enrollment and participation in referred assistance programs

Relationship & Program Support

  • Foster caring, respectful, and supportive relationships that encourage healthy decision-making and family stability
  • Encourage personal growth, life skills development, and future planning
  • Assist with mentoring initiatives, support groups, and post-birth care programs
  • Help clients build confidence, resilience, and healthy coping strategies
  • Collaborate with clinic leadership to identify barriers impacting client success and help develop solutions

Administrative & Ministry Support

  • Maintain accurate and timely documentation and reporting
  • Collect and track program metrics and grant-related data
  • Record referral calls and applicable reimbursement information
  • Attend team meetings, staff meetings, trainings, and organizational events
  • Assist with community outreach, church presentations, educational events, and fundraising initiatives as needed
  • Build positive working relationships with staff, volunteers, and community partners

Required Qualifications

  • Committed Christian with a personal relationship with Jesus Christ as Savior and Lord
  • Strong commitment to the sanctity of human life and biblical sexual ethics
  • Agreement with The Source's Statement of Principle, Statement of Faith, and organizational policies
  • Bachelor's degree in a related social services field required
  • Minimum one (1) year of experience in case management, care coordination, coaching, mentoring, client advocacy, or related support services
  • Strong organizational and communication skills
  • Ability to handle sensitive situations with compassion, professionalism, and discretion
  • Proficiency with Microsoft Office applications and database systems
  • Ability to work effectively in a fast-paced environment
  • Dependable, self-motivated, and team-oriented


Preferred Qualifications

  • Master of Social Work (MSW) with Texas licensure
  • Registered Nurse (RN)
  • Certified Community Health Worker
  • Training or certification in motivational interviewing, mentoring, coaching, therapeutic relationships, or adult/teen education
