Walworth County, WI
Full-Time | Mโ€“F, First Shift | Hybrid Work Option for Wisconsin residents.

Lutheran Social Services of Wisconsin & Upper Michigan is seeking a dedicated Care Coordinator to join our CLTS Waiver Program team in Walworth County!

The Childrenโ€™s Long-Term Support (CLTS) Waiver Program is a federally funded Medicaid initiative that helps children with developmental, physical, or severe emotional disabilities receive services that allow them to live safely in their homes and communities. As a Care Coordinator, youโ€™ll be the primary point of contact for families, helping them access and navigate essential services with compassion and care.

What Youโ€™ll Do
  • Assess childrenโ€™s functional abilities using approved tools
  • Develop and implement individualized service plans with families and providers
  • Facilitate team meetings and coordinate services based on family-centered goals
  • Maintain accurate documentation and meet all regulatory timelines
  • Collaborate with internal teams and external agencies to support families
  • Testify in legal proceedings when required
  • Communicate clearly with families, providers, and team members
  • Participate in staff development, training, and supervision

Qualifications
