Heartline Staffing is recruiting a Primary Care Physician to provide value-based healthcare to Medicare patient populations. Physicians practice at a single medical center supported by large care teams, allowing providers to focus on quality of care rather than volume of services. The value-based care model is structured around health outcomes, preventive medicine, low hospital admissions, and patient experience.
POSITION OVERVIEW
• Primary care for Medicare patient populations
• Value-Based Healthcare model

COMPENSATION
• $174,070-$374,920 base annual salary range
• Actual base salary offer depends on experience, education, geography, and other relevant factors

SCHEDULE
• Full-time
• 40 anticipated hours per week
• On-site
ROLE
• Assess and diagnose patients
• Oversee, direct, and administer primary care
• Prescribe and administer pharmaceutical treatments and medication
• Maintain Patient Electronic Medical Record data using Canopy and Greenway
• Collaborate with regional and central leadership to meet health quality goals
• Work with Practice Managers to direct and manage the center care team
• Perform other duties as assigned

Requirements:

Requirements
• Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) graduate REQUIRED
• Internal Medicine or Family Medicine Board Certified or Board Eligible REQUIRED
• Active, non-probationary, unrestricted state license REQUIRED
• Active DEA license REQUIRED
• U.S. work authorization REQUIRED
Preferred
• Fellowship training in Geriatrics
• Experience practicing medicine among geriatric populations
• Experience operating in a Value-Based Healthcare Model
• Experience working in a collaborative setting to ensure positive health outcomes
• Experience in outpatient primary care settings
• Bilingual proficiency where applicable
