CareDiem Home Health is seeking compassionate and reliable RNs to provide skilled home health nursing services to patients in CHICAGO, IL and surrounding suburbs. Our mission is to help clients remain safely at home while receiving top-quality, personalized care.

What You’ll Do:

  • Deliver skilled nursing care under a physician’s order

  • Complete assessments, medication administration, and wound care

  • Monitor and document patient progress using our electronic charting system (Axxess)

  • Educate patients and families on health conditions and care

  • Communicate with physicians and the care team

  • Travel to patient homes

What We’re Looking For:

  • Current RN license in the state of Illinois

  • 1+ year nursing experience (home health preferred)

  • CPR certification (BLS)

  • Reliable transportation, valid driver’s license, and auto insurance

  • Strong clinical judgment, communication, and independence
