Are you passionate about helping individuals successfully begin and navigate their recovery journey? LifeSpring Recovery is seeking an organized, compassionate, and detail-oriented Intake Care Coordinator to serve as the central point of coordination from admission through discharge planning.

In this role, you'll help ensure every client receives a seamless, supportive, and well-coordinated treatment experience while working alongside an interdisciplinary team dedicated to changing lives.

Position Overview

The Intake Care Coordinator plays a critical role in coordinating each client's treatment journeyfrom admission through discharge planning. This position serves as the communication hub between clients, medical providers, clinical staff, nursing, recovery support, utilization review, referral sources, and community partners to ensure continuity of care, regulatory compliance, and an exceptional patient experience.

Depending on operational needs, the Intake Care Coordinator may focus primarily on admissions, discharge planning, or both.


Essential Duties & Responsibilities

Care Coordination

  • Serve as the primary communication link between clients, treatment teams, referral sources, and community providers.
  • Coordinate services throughout the client's treatment episode to ensure continuity of care.
  • Maintain timely, accurate documentation of care coordination activities.
  • Assist clients in developing recovery goals and connecting with appropriate internal and community resources.
  • Monitor payer authorizations and collaborate with Utilization Review to support continued stay requirements.
  • Communicate changes in client needs with clinical, medical, nursing, and recovery support staff.

Intake & Admission

  • Coordinate the admission process and complete required intake documentation.
  • Conduct initial screenings and biopsychosocial assessments within the scope of licensure or credentialing.
  • Orient new clients to the facility, program expectations, participant handbook, client rights, and treatment process.
  • Collaborate with the interdisciplinary treatment team to establish initial treatment goals.
  • Ensure all admission documentation, consents, assessments, and electronic medical record entries are complete and accurate.
  • Coordinate physician orders, laboratory work, and medical evaluations.
  • Provide a welcoming, trauma-informed admission experience.

Discharge Planning

  • Begin discharge planning at admission and update plans throughout treatment.
  • Coordinate referrals to outpatient providers, MAT providers, sober living, housing resources, peer recovery services, and community agencies.
  • Review discharge instructions, medications, appointments, and safety planning with clients.
  • Complete discharge summaries and required documentation within established timelines.
  • Facilitate warm handoffs and communication with receiving providers.

Crisis Intervention

  • Recognize behavioral, emotional, and medical concerns requiring immediate intervention.
  • Follow established de-escalation and crisis response procedures.
  • Collaborate with clinical and nursing staff during crisis situations.
  • Document crisis interventions accurately and timely.

Compliance & Quality

  • Ensure compliance with Joint Commission standards, state licensing regulations, payer requirements, HIPAA, and 42 CFR Part 2.
  • Participate in quality improvement initiatives and chart audits.
  • Maintain confidentiality and professional boundaries at all times.
  • Perform other duties as assigned.


Qualifications

Required

  • Bachelor's degree in Social Work, Psychology, Human Services, or a related field; or
  • Minimum of two (2) years of experience in a behavioral health or substance use disorder treatment setting in lieu of a degree.
  • Minimum of two (2) years of experience in behavioral health, substance use treatment, case management, care coordination, admissions, or discharge planning.
  • Strong organizational, communication, and documentation skills.
  • Current CPR certification or ability to obtain within 30 days of hire.
  • Ability to pass all required pre-employment screenings.

Preferred

  • Licensure or certification in a behavioral health discipline (LBSW, LMSW, LMLP, LPC, LAC, or similar).
  • Experience with ASAM Criteria.
  • Experience with electronic medical records.
  • Knowledge of Joint Commission standards.
  • Experience coordinating care within a substance use disorder treatment program.


Work Environment & Physical Requirements

  • Work is performed in an indoor behavioral healthcare environment.
  • Frequent sitting, standing, walking, and computer use.
  • Occasional lifting of up to 25 pounds.
  • Frequent interaction with clients experiencing substance use and behavioral health conditions.
  • Ability to respond appropriately during crisis situations.
  • Adherence to infection prevention and workplace safety practices.
