Now Hiring: Nurse Case Manager (RN) About the Role: The Nurse Case Manager is a vital part of our community. You will be managing a diverse caseload of skilled nursing residents. Your role focuses on providing comprehensive medically related social services, ensuring residents attain or maintain their highest practicable physical, mental, and psychosocial well-being. You will also be responsible for navigating residents to the appropriate services, acting as a connector to ensure their needs are met through various community resources, ensure their discharge is set up for success and support systems and assisting the team to help residents with apply for Medicaid.


Key Responsibilities:

• Attend required meetings

• Assist with new admission chart audits

• Support the rehab unit and assist with admissions and discharges.

• Coordinate delivery of services to managed care and Medicare residents in collaboration with the facility’s team members.

• Complete timely and thorough communication with managed care/insurance case managers to coordinate certification and stay programs.

• Participate in the orientation of new residents/family members to the facility.

• Responsible for scheduling care conferences with residents, family members, and staff.

• Participate in developing, implementing, and periodically updating the resident’s written discharge plan.

• Rotating on call for floor coverage and clinical triage

• Floor coverage as needed
