Key Responsibilities
Serve as the attending physician for patients in each assigned SNF
Collaborate with NPs/PAs to manage daily patient care and oversee clinical supervision
Diagnose and treat acute and chronic conditions, delivering high-quality, patient-centered primary care
Participate in Chronic Care Management, Transitional Care, Remote Patient Monitoring, and other quality initiatives
Qualifications
  • MD or DO with an active U.S. medical license
  • Board-certified or board-eligible in an appropriate specialty
  • New graduates who are board-eligible and actively pursuing licensing are encouraged to apply
